At the time of the visit there was not a 6 foot fall zone on all sides of the dome climber on the preschool playground. There was a playhouse in the fall zone of the climber on the preschool playground. The toddler structures did not have the 6 foot fall zones on all sides. All of the climbers did not have 9 inches of mulch in all areas a child could fall from.

Attendance not accurate in two classrooms.
One classroom was not using a 2-step process to clean and disinfect. Educators stated they use soap and water on tables prior to children eating and only bleach and water is used on diaper changing surface.

Health and safety inspection found the following: multiple educators had left their personal bags and drinks within reach of children and a rusted heater vent was found in a toddler room.

At the time of the visit the cords for the mini blinds were hanging and accessible to the children. The climber in the Toddler classroom did not have gym mats at the bottom of the slide and the stairs.
