First Aid Kit (FCC)
Licensor randomly reviewed 4 of the children's records. Of these, 1 was complete and uo tp date, but the other 3 were all missing a signature on pg. 6 of the enrollment packet, under Basic Transport.
The Parent Handbook is missing several sections, such as emergency preparedness plan, lead disclosure, mandated reporter, use of assistant, and several more.
All of the required items were observed in the 1st aid kit, except for tweezers. The 1st aid kit is also missing 2 of the children’s emergency contact forms.

The emergency preparedness plan is posted in the program, but it does not include the plan for if a child goes missing from the program.
There was one outlet that was not covered in the dining room.
Licensor inspected all of the indoor, licensed space. The only hazards observed were in the kitchen. The drawer on the left side by the coffee maker had choking hazards and a screwdriver inside, and there was a lighter on the counter.

Licensor inspected all of the licensed, indoor space during the visit. The following hazards were observed: ** LIVING ROOM **- in the desk, on the left side, there were small chokable items such as tacks in the second drawer down, ** SUN ROOM **- tall shelving unit is a tipping hazard as it is not secured to the wall ** KITCHEN **- cleaners are stored in a cabinet under the sink. This cabinet has a child safety lock. However, the lock was not engaged during the visit. Also, the 3rd drawer down to the right of the stove has long wooden skewers in it, which are a hazard, ** DINING ROOM **- no hazards. ** BASEMENT: PLAYROOM **- no hazards ** NAP ROOM **- the cord to a karaoke machine was plugged in and hanging down from a high shelf. This is a hazard because if a child pulled on the cord, the karaoke machine would fall off the shelf. BATHROOM - no hazards OUTSIDE - no hazards

Licensor randomly reviewed 3 of the children's records. Of these, 2 were complete and up to date. The third record had out of date lead testing.
All of the required items were observed in the first aid kit, except for an instant cold pack.
Educator has the emergency numbers posted on a bulletin board in the basement playroom. However, Educator needs to add her own contact information, including her name, address, and phone number.
When Licensor arrived at the program, Educator was on the 1st floor preparing lunch and the children were in the basement space. During the visit, Educator again went to the 1st floor to finish lunch and left the children in the basement space. Discussed that Educator is required to remain on the same floor level as the children at all times. If she needs to go to the first floor, all of the children need to go with her.
