CPR/First Aid
Educator was not able to locate her CPR/1st Aid certification. Educator stated she would email the document, but Licensor has not received that email as of the writing of this report. Educator is a trainer for CPR courses.
All of the children's progress reports are out of date.
All of the required items were observed in the 1st aid kit, except an instant cold pack.
The emergency numbers/information are posted by the stairs. However, Educator needs to add her address.
Licensor inspected all of the licensed, indoor space during the visit, as well as the outdoor playspace (fenced in back yard). The only hazards that were observed were an outlet that was not covered in the family room and the activity room.
Educator stated that she did not have progress reports for the childcare children.

Of these, 1 had out of date signatures in the enrollment packet and an out of date physical, 1 had no physical and out of date signatures in the enrollment packet,1 had out of date signatures, but an up to date physical, and the final record was complete and up to date. Please review all of the children's records and ensure they are all complete and up to date.
Educator's evacuation log is out of date as no fire drill was documented for April, May , or June.
During the visit, Educator was not able to provide Licensor with the children's progress reports, stating they were on another floor. Educator stated she would email the reports to Licensor after the visit. However, Licensor did not receive any email with progress reports. **Please ensure that all of the children enrolled in the program have a complete and up to date progress report.

Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. No hazards were observed inside the home. The only hazard outside was that the grill was not covered.
Licensor was able to inspect all of the licensed space during the monitoring visit, as well as the outdoor playspace. No hazards were observed in any of the licensed rooms in the home. Outside, the only hazard was a plastic slide that is over 18 inches and did not have a fallzone.

There is one child enrolled who is [REDACTED] , so Educator has ensured that there are none in the program. However, Educator does not have this information posted in the program. Discussed the need to post this information in a visible area and then cover it with a sheet to ensure the child's confidentiality. Educator stores the [REDACTED] in the child's individual bin on a high shelf in the kitchen.

Licensor was able to inspect 3 of the children's records during the virtual visit. All 3 of these records had out of date signatures in the enrollment packet, and out of date [REDACTED]. One of the records also had an out of date progress report. One of the records was also missing a signature for Providing Transportation. This is especially important as this is one of the children who is transported to school by Educator. Licensor explained that this child cannot be transported until this signature is obtained from the parent. Please review all of the children's records and ensure that they are all complete and up to date. Also, please provide a plan for how you will ensure that these records remain up to date moving forward.

There were four children present in Educator's program during the monitoring visit, ages 5 months, 16 months, 2.5 years old, 3 years old, 4 years old. However, when Educator brought 3 of the childcare children to another FCC program, there were 8 children present. when Educator left that program to pick up children from preschool, that left the other Educator alone with 8 children, none of whom were school age, so the program was over capacity. Then, when Educator returned with 2 additional children, there were 10 children in the program, which is over the other Educator's licensed capacity of 8 children.

The first aid kit was missing a couple of items, including the tweezers, CPR mouth guard, and gauze pads.
Educator was not able to locate the Emergency Preparedness Plan during the virtual visit. Educator was asked to email the plan to Licensor, but as of 11/8/20, the plan had not been emailed.

CPR/First Aid - issued 10/14/18. Educator's CPR is expired. Educator stated she has an updated certification, but was not able to locate it during the monitoring visit. If the certificate is located, please upload it to the corrective actions. If the certificate is not located, Educator has 7 days to take a new CPR course and up date her certification.
Progress reports were not available during this visit. Please upload verification that a current progress report has been completed for all required children.
When Licensor arrived at the program, no one was home. Licensor called Educator and learned that Educator was across the street at another licensed FCC program, with 3 of her childcare children. Licensor then walked to the other program a few minutes later. Educator had left the home, leaving her 3 childcare children with the other FCC provider. Licensor was informed that Educator went to go pick up another child from preschool.

Licensor walked through all of the licensed space and observed the following hazards: ** Playroom - no hazards ** Activity room- no hazards ** Playroom- no hazards ** Bathroom- trash does not have a cover ** Outdoor- sand area has no cover.
