At the time of the visit an infant was observed sleeping with a blanket.
During the visit the following was observed. Equipment throughout the center was tippable and not secure.
The large yellow climber had an area of one of the upper decks had a large gap a child could fall through. Gaps under the fence on the playground were observed.
At the time of the visit, it was found that there was a new center Director and EEC had not been notified.
This follow-up visit inspection recorded no violations or advisories.
During health and safety inspection educators personal drinks and purses were left on floors and shelves within reach of children which pose potential hazards.

Not all evacuation drill log documentation could be found during the visit so it could not be determined if drills are conducted monthly.
Educator in the PreK classroom was not aware there was a child in the classroom that had a chronic medical condition that requires emergency medication.

A review of staff records found that one staff file reviewed did not have dates on their bimonthly observations.
At the time of the visit some equipment that was tippable was not secure. Please ensure that any piece of equipment that could tip over is secured.
At the time of the visit the following was observed. Swing structure for a saucer/web swing to attach to was tippable and did not have a saucer swing attached. There were smaller pieces of equipment in the fall zone of the "jeep" on the toddler playground. A small multicolor platform was in the fall zone of the little tykes climber on the toddler playground.
At the time of the visit there was a bottle of bleach on the counter in the men’s bathroom accessible.
Toxic Substances
One staff member had an outdated physical.
At the time of the visit there was a bottle of bleach and a bottle of hand sanitizer accessible to the Children in one of the bathrooms.
The little tykes play jeep had a picnic table in the fall zone. The platform climber at the back of the playground did not have the six foot fall zone on all sides. At the time of the visit there were weeds and vines growing through the fence. Please ensure that the area around the fence is kept free of debris.

At the time of the visit there was not the required 9 inches of mulch in all areas of the playground that contains climbing structures. At the time of the visit there was a web swing that did not have any impact absorbing material under and around the swing area.

Random review of 5 staff files found 1 without a staff information form and 1 with only 1 reference.
4 infants were being fed at the same time. One educator held one infant while sitting on the floor next to another. Other staff person sat in the rocking chair feeding an infant and other infant was left alone on the floor drinking their bottle while laying on a boppy.
There were 6 children in the infant classroom and only 4 were marked in on the attendance log.
Infant educators reported getting hot water in a mug from kitchenette area and bringing to the classroom to warm bottles. Program cited last year for improper bottle warming procedures.
Health and safety inspection found a dirty area rug in infant classroom and the side of sink in PreK is peeling off the particle board.
Licensor had to remind educators in two classrooms to wash children's hands after diapering.

Random review of 8 children's files found 1 file that did not have all enrollment forms dated therefore it could not be determined if it was current.
Random review of 5 staff files found 1 without a current physical and 1 with no references, no first aid and no staff information form.
During observation the PreK class had 10 children but only 6 were signed in on the attendance sheet. An 11th child arrived and the educator moved one child to the Preschool classroom to remain in ratio. Educator did not document new child's arrival or mark out the child that was transitioned into the Preschool room.
During the visit it was learned educators in the infant classroom heat water in the microwave to warm infant bottles.
A child in the PreK classroom had emergency medication but the educator could not locate the medication.
Two-step process to clean and disinfect on required surfaces not always used. An educator stated they only use soap and water on tables prior to children eating.
Toxic Substances
At the time of the visit the following was observed. Home Living area furniture was tippable and not secure in most classrooms. Plunger in the bathroom accessible to the children. Bleach and hand sanitizer accessible to the children in one bathroom. Ceiling in the downstairs boys bathroom had a hole and there was visible water damage. Walls throughout the center in areas had peeling paint. Molding in the preschool and Pre- K classrooms had come off and the wall is in disrepair.
Program had chipping and peeling paint throughout the facility.
