Evacuation procedures posted at each exit
At the time of the visit one staff member's file reviewed had an expired BRC. Staff member was not at the program on the day of the visit. Staff member cannot return to work until BRC is complete.
At the time of the visit the classroom upstairs did not have evacuation procedures posted at each exit.
This follow-up visit inspection recorded no violations or advisories.

A review of documents in the document library found that the Fire inspection indicated that facility did not pass the inspection. Please submit a Fire inspection without violations. There was no building inspection uploaded only a paper that stated the facility passed.
At the time of the visit there was an expired [REDACTED] for a child with an allergy. At the time of the visit there was a bottle of Benadryl that was expired.

A review of 3 children’s files found all 3 files were missing transportation form.
Staff Records - R Apr 2025
1 file missing an updated physical exam.1 staff did not have evidence of an updated PQ Registry in her file.
One staff file did not have evidence of a physical,immunity to[REDACTED] EEC strong start trainings and an updated PQ registry. Ongoing bi-monthly observations were not completed for any staff over the past year.
The mat used for diapering children was being stored on the bathroom floor.
The shed/barn has chipping paint accessible to the children.
At the time of the visit the fence was in disrepair in some areas and the double gate had entrapment areas. The wooden shed/barn had rotted wood/splintering wood accessible to the children. There was a Jumperoo on the playground that did not have the proper fall zone.
At the time of the visit documentation of monthly evacuation drills was not available.
At the time of the visit there was an expired [REDACTED] for one child.
At the time of the visit a child was observed blowing her nose and did not wash her hands with soap and running water after.
A review of individual healthcare plans found that one child’s IHCP was not signed by the child’s parent.
