Current consent for administered medications
At the time of the visit, in a review of three educators files, the documentation was not fully complete. Two educators were missing a staff information form, and verification of two references. Educator A was missing documentation of a current bi-monthly observation. Educator B was missing documentation of an annual evaluation. Educator C was missing documentation of two MMRs. The program must maintain a personnel record for each staff member as outlined by the Staff Records Checklist.

At the time of the visit, in a review of three students files, the documentation was not fully complete. All three students were missing documentation of the Transportation Plan. Student A was missing documentation of a face sheet. Student B had expired enrollment paperwork. Student C had an expired physical. Children's records must have all required applicable documentation as outlined by the children's record checklist.
At the time of the visit, the location of the health care policy and the location of the first aid kit were not posted. The licensee must post the following information in an area easily visible to parents, educators and visitors: the location of the health care policy and the location of the first aid kit.
At the time of the visit, the allergies were posted in the directors office in a manner that did not protect the privacy of the student. A list of all emergency or life saving medications, that specifies to which children they belong; and a list of allergies and/or other emergency medical information provided by the parent for each child mut be posted in a manner that protects the privacy of each child.
At the time of the visit, in a review of one students IHCP and the corresponding medication consent documentation, the program was missing documentation of medication consent for two prescribed medications outlined in the IHCP. All medications must be accompanied by written parental consent and healthcare practitioner authorization when required, including proper administration of medication.
At the time of the visit, emergency/ life saving medication was stored within the office. Emergency or life-saving medications, for any children for whom they have been prescribed must be readily available, including within the classroom and when outdoors.

One exit doorway was locked from the inside. This non compliance was cited on the previous visit also.
One group of five children were outside in the playground with one educator; however, there was not an attendance list with the group. Each group must maintain an accurate attendance at all times.
One white wooden cubby shelf, and one wooden play store, were tippable not secured.
Two exit doors were locked from the inside; this could prevent a safe evacuation in the event of an emergency. The doors were unlocked during the visit.
Hazards
A review of one ch ild’s record indicated that the Individual Health Care Plan (IHCP) was not completed.

One container of Lysol wipes was accessible to children in the bathroom. This was moved at the time of the visit. The cord for the CD player was draped in a manner accessible to children.
