At the time of the visit, in a review of the fire drill log the documentation was not fully complete. Documentation of the September, November and December 2025 and January 2026 drills were missing. The program was previously cited for missing fire drill documentation in 2/2025. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.

Evacuation procedures posted at each exit
At the time of the visit, in a review of four educators files, the documentation was not fully complete. Two educators had an expired PQR and one educator was missing documentation of a PQR. Educator D had an expired physical. The program must maintain a personnel record for each staff member as outlined but the Staff Record Checklist.
At the time of the visit, it was identified that the programs First Aid/ Emergency Medical Consent & Release (within all students files) was missing documentation as required by EEC. Children's Records must be maintained as outlined by the children's records checklist.
At the time of the visit, it was observed that an educator within the infant classroom left the classroom space, and walked to the sink in the I/T classroom attached to the room. At the time of this observation, the three infant students were sitting within highchairs; the three students were in the room without an educator for roughly five seconds. Educators must be positioned to maximize their ability to see and/or hear children in their care. Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children.
At the time of the visit, it was observed that the program had evacuation routes posted but were missing evacuation procedures within the lobby and infant classroom. The program was previously cited in 2/2025 for missing evacuation procedures throughout the building. Written emergency evacuation procedures must be posted next to each exit.
This monitoring outside differential licensing cycle visit inspection recorded no violations or advisories.
At the time of the visit, in a review of three educators staff files, the documentation was not fully complete. Two educators were missing current documentation of professional development. One educator was missing documentation of staff orientation. Two educators had expired physicals and one educator did not have a current staff evaluation on file.

At the time of the visit, it was observed that two educators responsible for ratio within the Toddler Classroom were both Assistant Teachers. At the time of the visit they had nine students within the classroom. The licensee must maintain sufficient numbers of qualified staff to promote the health, safety, growth and development of each child. The program must maintain staff to child ratios as outlined in regulation. The program must have the number of educators necessary to ensure adequate supervision of the group at all times.

A walk through of the facility indicated the following: -In the PreK class, there was a tippable bookshelf that needs to be secured. -In the Yearling class, there was a tippable play kitchen that needs to be secured. -In the Yearling class, there were rips/tears in the changing table pad. The changing pad was not covered.

The Infant 1 classroom attendance indicated three children were present, but there were 5 children in the classroom. When educators were asked about this, they stated that one child was visiting from another classroom and the other child had recently arrived. -The Infant 2 classroom attendance listed one child as "out" but did not indicate the time the child was signed out of the classroom. Attendance records must document arrival and departure times.
Baseboard heating in the Toddler Room was not protected. TA was provided.
Adult scissors were accessible to children in the T/PS Room; the scissors were removed during the visit. A wire more than 6 inches, connected to an iPad on a shelf, was accessible to children. On the ramp leading to the playground two pieces of floor trim wood, were accessible to children. The door to the adult bathroom was not locked, in the room were 2 cans of aerosol spray, accessible to children. TA was provided.

Many baby products for example Aquaphor and Butt Paste were accessible to children under the diaper tables in the TODDLER room and the KIDS BATHROOM. These products are considered hazardous as they have labels stating "Keep out of reach of children".
Only doors directly opening to the outside of the building had evacuation routes posted. None of the other doorways had diagrams with escape routes. Reg 7.11 (18)(b) "Escape Routes. A diagram of escape routes must be posted conspicuously at each means of egress in rooms that do not have direct access to the outdoors."

The Assistant Teacher in PRESCHOOL PREP was unaware of individual health needs of the children in the classroom. When asked, she looked to the Lead Teacher to answer when asked if any of the children had health needs.

At the time of the visit, in a review of three students files one student was missing documentation of off site consent, a physical and led screening. The licensee must maintain an individual written record for each child that includes all required applicable documentation according to the Children's Record Checklist.
At the time of the visit, it was observed that educators within the Toddler and Yearling classrooms were leaving ratio to heat up students lunches. It was observed that an educator within the Preschool Classroom left ratio to get supplies within the Infant classroom. During all observations, one educator was left in the Toddler, Yearling and Preschool classroom over ratio. Educators must be positioned to maximize their ability to see and/or hear children in their care. Educators must not engage in any other activities or tasks that could unnecessarily divert their attention from supervising the children.
At the time of the visit, it was observed that a play refrigerator within the PreK classroom, a play kitchen within the Yearling classroom and a bookshelf within the Toddler classroom were not secure and were tippable. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. The equipment used in the program must be sturdy, safely constructed and installed and non-tippable.
At the time of the visit, it was observed that the program had evacuation routes posted at all exits but did not have evacuation procedures posted. Written emergency evacuation procedures must be posted next to each exit.
At the time of the visit, it was observed that the Emergency/Fire Drill log was not fully complete. Evidence of December 2024 and January 2025 drills were missing. Documentation of the number of students completing the September 2024 drill was missing. The program must document the date, time, exit route used, number of children evacuated and effectiveness of each monthly drill.
At the time of the visit, it was observed that an educator within the Infant Classroom did not wash two students hands after completing diaper changing. The educator must ensure after changing, the child's hands are washed with liquid soap and water, and dried with individual or disposable towels.
At the time of the visit, in a review of one students IHCP, the documentation was not fully complete. The documentation was missing authorization from a medical provider and the description of the chronic illness. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner.
The program did not have evidence of a monthly fire/evacuation drill for the January 2024.

There was not an attendance list for children in the Toddler/Preschool Room; eight children were present. Attendance in the Preschool Room was not accurate; 18 children were present, 17 were listed on the attendance list.
On the playground, metal poles in cement, were not secured and accessible to children.

There was not a current staff schedule posted at the program. The Licensee must establish a written schedule listing all persons on duty, including volunteers. The schedule must show compliance with at least the minimum educator ratios at all times, including during breaks and planning time;

Fencing around the garden, on the playground, measured 4+ inches between posts, resulting in an entrapment hazard. TA was provided.

The binder containing emergency drills was assessed and missing the following months: October 2020 through September 2021 and June 2022. Effectiveness of the drills were also not noted on the drills conducted (time lapsed and any notes).

Four child records were reviewed. One does not have evidence of a physical within the past year nor evidence of a lead screening or progress reports. The second child has missing information on the IHCP (who can administer and who trained staff). The third child was missing information on the IHCP (who can administer and who trained staff), the parent signature was missing from the developmental history, progress reports, and a medication consent form signed by the parents within the past year. The four child record was missing a progress report.

Two of the three staff records reviewed did not have evidence of First Aid certification.
As described above, the Lead Teacher left the Assistant Teacher alone with seven infants in the INFANT A classroom. The ratio for this age group is 1:3 and one additional educator for 4-9 children.
The current staff schedule has not been provided.
An Assistant Teacher in the INFANT A classroom was alone with seven infants when Licensor entered the classroom. The Lead Teacher returned within a minute of the Licensor entering. Assistant Teachers must be supervised at all times while caring for children.
Both the Building and Fire Inspections are overdue.
A portion of the fence is no longer secured at the ground and can be moved by hand, which when lifted, creates an entrapment hazard space. Licensor and Director discussed fixing this by adding a piece of wood near the bottom of the fence and securing the fence to it.
Paint has begun peeling on the long wall in the TODDLER CLASSROOM and is accessible to children. They were each about the size of a quarter.
Emergency phone numbers and information was not visible to Licensor in any classroom or in the office off the Preschool Prep room. Director reported the only list of emergency numbers is located at the front of the building near the door but upon discussing the protocols in an emergency, because staff are expected to call 911 directly in an emergency, the list of information and numbers should be posted in each classroom.
