At the time of the visit, in a review of one students IHCP and the corresponding prescribed medication, the medication did not have the prescription label attached. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer's packaging.

At the time of the visit, in a review of one IHCP and the corresponding medical documentation, the Medication Consent was not fully complete. The Medication Consent documentation was a form from the public school and was missing documentation on if the medication was a prescription, if the student had taken the medication previously, the dates for the medication to be provided, the times for the medication to be provided, the reason for the prescribed medication, and the directions for storage. All medications must be administered in accordance with written parental consent and documentation requirements specified.

A review of the staff meeting log indicated that not all staff meetings were held for at least 2 hours as required by EEC.
A current EEC License was not posted. The license must be posted in an area easily visible to parents, educators, and visitors.
In a review of two Individual Health Care Plans, documentation of side effects of the prescribed medication was missing on both, one was missing documentation of who trained the educators and what educators were trained on the condition. One student had expired Benadryl and the other student had expired Albuterol.
The Fire Inspection was not available on the date of the visit.
The posted allergy list ( Red Flags) was not current; two children were listed who no longer attend.
There was not a parent/guardian consent to administer medication, in this case an (REDACTED), for one child.
There was not (REDACTED) available for one child with a diagnosed allergy; although the current physical exam indicates that this antihistamine must be used in the event of an exposure/reaction.
