Notification to EEC
During a review of the case stating there was a “slight carbon monoxide detected in the ceiling” (on 1/5/2026), three classrooms (Infant 2, Toddler 1 and Toddler 2) were changed without notifying EEC. Infant 2 moved to the Motor Room. Toddler 1 and Toddler 2 rooms were moved into the Preschool 1 classroom. The Director stated all groups moved curriculum materials to the new spaces and the Preschool 1 room was separated into two spaces using furniture to separate the two toddler groupings. The licensee must notify the Department when possible, at least 30 days prior to any change in the space used by the program. If the licensee is unable to provide 30 days notice, the licensee must notify the Department by telephone immediately upon learning of the impending change.
Change of Space
The program submitted two cases via LEAD on 3/30/2026, notifying the Department of two incidents (from 12/29/2025 and 1/5/2026) that involved the Fire Department responding to the program and being on the premises. In the event of an incident on the child care premises (other than a false alarm) that results in a response by the fire department, the licensee must notify the Department by telephone within 24 hours of the incident. The licensee, if requested by the Department, must prepare and submit to the Department a written report regarding the incident.

Individual Health Care Plan (IHCP)
During the visit, one of the educators in a Preschool classroom was heard shouting very loudly and in an angry tone at a couple children in the classroom to redirect them. Licensor and Program Director heard this and walked over to the room. Program Director stepped into the classroom and supported the children prepare their cots for rest time.
The following was observed during the visit to be accessible to children: In TODDLER 1, the left side of the sink cabinet was peeling and the surface underneath the changing pad had visible dirt and crumbs and in PRESCHOOL 1, the cabinet between the two windows was unlocked and the right side contained loose bags and an open box of gloves (strangulation hazard).

One water bottle in Toddler 1 and one water bottle in Toddler 2 were not labeled with the children's names.

The following was observed to be accessible to children: the storage closet near the rear exit door off the main lobby was unlocked and contained hazardous materials such as a can of WD-40 and cans of spray paint (sitting on the floor) with the labels "Keep out of reach of children"; and in the PREK bathroom, a plunger was in the bathroom; in TODDLER 2, multiple rips were visible on the couch; in PREK a staff pocket book sat under an unlocked cabinet; and in the walkthrough/hallway space outside of the toddler classrooms, the cord for the air purifier that sits on the cubbies was long and loose (strangulation hazard).
Electrical Outlets
A Safe Sleep violation was cited today on the visit. The following was observed in the INFANT 1 classroom, a sound machine was hanging from the top of a crib with a baby sleeping inside. The EEC Safe Sleep Policy was provided to the Director and the two educators working in the classroom at the time were assigned to take the Safe Sleep training on StrongStart. Hanging objects such as mobiles, crib toys, or mirrors that can be reached by the infant are not allowed. Areas used for infant sleep must be free of hazards, such as dangling cords, electric wires, and window-covering cords, and any potential strangulation risks.

Emergency Evacuation - R Apr 2025
One children's IHCP was not current and expired on (REDACTED). Another children's allergy action plan from the doctor was not current and expired on (REDACTED).
The emergency postings were in each classroom but they were not posted in a manner that protects the children's information and one was incomplete. One posting did not include that two children had asthma and inhalers to treat.(c) in a manner that protects the privacy of each child: 1. a list of all emergency or life saving medications, including but not limited to epinephrine auto-injectors, inhalers, and anti-seizure medications, that specifies to which children they belong; and 2. a list of allergies and/or other emergency medical information provided by the parent for each child.
Evacuation procedures were missing from the following: -In PREK, at the door leading to the Motor Room the plan was missing. -In PRESCHOOL 1, at the door in the back right corner of the room that leads to the hallway near the back exit of the building the plan was missing.
The interior door leading from the PREK CLASSROOM to the MOTOR ROOM was blocked on the MOTOR ROOM side by a storage container and was unable to be opened.

In the gross motor room, a surface disinfectant/ deodorizing spray was on the window sill, which is accessible to children. --In the foyer to the PS entry: the overhead ceiling tile support rods were rusty.

One preschool room indicated that they had 14 children but then looked at the attendance record and indicated she had 15 children.
The licensee must not use any equipment, materials, furnishings, toys, or games identified by the U.S. Consumer Product Safety Commission as being hazardous. The following were observed indoors: -Chipped paint throughout the program -A patch of Peeling tape in one preschool room. -heating lamps were accessible to children and the cords were not out of reach. -there was one trash can that had food wasted that was uncovered.

Five child records were reviewed during the visit and found: two children's records were missing current physicals and another child's record was missing the doctor's allergy action plan.
Paint was observed during the visit to be chipping in the following areas: In PRESCHOOL 1, the dark blue wall on the wall with the phone; in TODDLER 2, the green wall had many little spots of chipped paint, and in TODDLER 3, the gray walls had many little spots of chipped paint. The licensee must maintain the interior and exterior of the program in good repair, free of chipping, flaking, or peeling paint or broken plaster.
The medical needs of children were posted, however, one child's name, chronic condition and medication was missing.
IHCPs were not complete. They were missing the names of educators trained to administer treatment.
One child's record is missing two medication consent forms for two medications.

Three staff records were reviewed and it was found: One staff record had expired PQ registration (expiration date is 11/16/2024), was missing evidence of an interview, evidence of two reference checks, a resume or application, a current physical (last dated 12/1/20222) and their orientation checklist was not signed by an administrator.
The Staff Meeting Binder was observed and it was found that evidence of staff meetings was missing for July and December 2024.
The Health Care Consultant Agreement expired 5/9/2024.
Six children's records were observed and it was found that one record had expired enrollment form, developmental history, and CPR/First Aid consent (expired 4/1/2025).
The following was observed to be accessible to children: In PRESCHOOL 2, the wooden bookshelf in the block area was not secure and tippable.
One outlet in PRESCHOOL 2 (near block area), two outlets in PREK (one in block area and one near the cubbies), and one outlet in the bathroom at the back of the program off of the lobby area were missing outlet covers. All electrical outlets within the reach of children younger than school age must be made inaccessible by use of a safety device or covering that prevents access to the receptacle openings. If the covering is a shock stop, it must be of adequate size to prevent a choking hazard.
The location of health care policy was not posted.
The PRESCHOOL 1 bathroom was observed to be missing diapering procedures.
The Building/Fire Inspection is current and expires 9/30/2025. The IPMs are not current and expired 7/2/2023.

Two staff records were reviewed and found: -One staff record was missing evidence of an interview and reference checks, CPR/First Aid certification, and bi-monthly observations since hired in April 2023. -The second staff record was missing evidence of reference checks.
The following was observed to be accessible to children: -In TODDLER 1 and TODDLER 2, wipes in bags were sitting on the open shelves in baskets under the changing tables. The bags now state to keep away from children due to suffocation. -In PREK, one of the unlocked lower cabinets contained a bottle of Air Fresher with the label “Keep out of reach of children” and an open box of loose bags and an unlocked drawer contained a package of thumbtacks. The thumbtacks were moved during the visit to be out of reach of children. -In the PREK BATHROOM, a piece of baseboard behind the toilet is peeling off and exposing plaster and dry wall. -In PRESCHOOL 1, an open box of gloves was sitting on the top of the gray shelf. This box was relocated during the visit. -In the PRESCHOOL 1 BATHROOM, a plunger was sitting next to the sink. -In PRESCHOOL 2, an open box of gloves was observed to be sitting on top of a shelf in the classroom and then another open box of gloves was found in the hall are near the bathroom next to a basket of loose bags. Gloves and loose bags are a suffocation and choking hazard.
A fan was observed to be on and in the TODDLER 3 CLASSROOM and sat on a low shelf. Electric fans, if used, must not be accessible to children.
One child’s emergency medication expired (REDACTED) and another child's antihistamine expired (REDACTED).
Two children have prescribed inhalers to treat asthma and those inhalers were kept in the program office and not readily accessible in the classroom.
