Staff First Aid Training
At the time of the visit, in a review of the current staff record checklist it was identified that Educator I was hired on 9/2/2025 and did not have documented First Aid Training. The Director identified the educator is a part time employee. The licensee must document that each educator is certified in first aid within six months of employment.

At the time of the visit, in a review of three IHCPs and the corresponding medication consent documentation, the information was not fully complete. One student was missing documentation of the dates for the medication to be provided. All medications must be accompanied by written parental consent and healthcare practitioner authorization when required, including proper administration of medication.

At the time of the visit, in a review of four students files, one student was missing documentation of a current progress report. For infants and children with identified special needs the progress report must be prepared every three months. For toddlers and preschoolers, the progress report must be prepared every six months. For school age children, the progress report must be prepared at least annually, at the midpoint of the child’s program year.
During the visit it was observed that several classrooms did not have proper protective covering for the diaper changing surface and were changing diapers without the proper covering.

It was observed during a diaper change that a teacher did not clean and then properly sanitize the diapering surface.
The refrigerators within the classrooms were missing thermometers.

The metal grate of one heating unit was detached from the wood frame. This left an exposed sharp edge and the heating unit potentially accessible.
The orange mesh barrier between the usable space and unusable outdoor space was not taught. Creating a potential trip or entanglement hazard.

At the time of the visit, it was observed that the Preschool Classroom had one educator alone with fourteen students. At this time, an Assistant Teacher had taken four students to the bathroom, which is located down the hall from the classroom. The program must have the number of educators necessary to ensure adequate supervision of the group at all times. The preschool ratio is one teacher to ten students. An Assistant Teacher must work at all times under the direct supervision of at least a teacher qualified staff person.
At the time of the visit it was observed that the toy fridge and toy oven within the Toddler B Classroom and the toy sink and bookshelf within the Toddler A Classroom were not secured and were tippable. The licensee must only use indoor and outdoor equipment, materials, furnishings, toys, and games that are appropriate to the ages, needs and developmental level of the children enrolled. They must be sturdy, safely constructed and installed, non-tippable, flame retardant, easily cleaned, and free from hazards that may be dangerous to children.
At the time of the visit, in a review of three IHCPs and the corresponding medications, one student was prescribed two medications for their diagnosed health condition. Both medications were outside the original box. The prescription medication was missing the prescription label. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer's packaging.

Throughout all classrooms in the program there was furniture that was tippable (shelves, dress up items, lockers).
Within the girls bathroom, a toilet paper holder was no longer adhered to the bathroom wall. A screw (from the toilet paper holder) was still on the wall and was protruding into the bathroom stall.
Two classrooms were missing evacuation routes by their second exit within the classroom.
During a diaper change, it was observed that the educator did not wash children's hands after completing the diaper change.
