Attendance Recording and Group Sizes/Ratios
At the time of the visit, it was observed that Child A was present within the PreK Classroom and was not enrolled in the program and was outside the age grouping on the programs license. Child B was not signed into the attendance within the PreK Classroom. The program must maintain accurate attendance and appropriate age groupings.

At the time of the visit it was observed that classrooms had evacuation routes posted but did not have evacuation procedures posted. The Rec Rooms did not have evacuation routes/ procedures posted. Please ensure all exits possibly used by the program have evacuation procedures posted and evacuation routes are posted at all exits as applicable.
At the time of the visit, it was observed that refrigerator downstairs was reading at 64 degrees F. At the time of the visit, the refrigerator did not have food items in it. The licensee must provide refrigeration and storage for food at no less than 32 degrees F, nor more than 40 degrees F for all food requiring refrigeration.
At the time of the visit it was observed that the First Aid supplies on site were missing a CPR mouthguard. The licensee must maintain adequate first aid supplies, including, but not limited to: adhesive tape, band aids, gauze pads, gauze roller bandage, disposable non-latex gloves, instant cold pack, scissors, tweezers, thermometer, and CPR mouth guard.

At the time of the visit the Toddler A and Toddler B classrooms had bookshelves that were unsecure and tippable. There was a shelf in the Preschool B room that was unsecure and tippable. At the time of the visit, it was observed that an outlet on the floor within the upstairs Rec Room had a broken outlet cover. Within the fenced in outdoor play area, the blots on the fence extended inward, towards where students would play. The indoor and outdoor equipment must be sturdy, safely constructed and installed, non-tippable, and free from any other hazards that may be dangerous to children.
At the time of the visit, it was observed that the location of the Health Care Policy was not posted. The licensee must post the following: the location of the health care policy.
