At the time of the visit, in a review of four students files, the documentation was not fully complete. Two students had expired off site consent documentation. Children's records must have all required applicable documentation according to the Children's Record Checklist.
At the time of the visit, in a review of the attendance record in the Infant Classroom, two students were signed in that had left the program. When the attendance was requested, one educator identified that two students who recently left were not signed out and they asked the director to sign them out. The program must maintain daily attendance records indicating each child's attendance, including arrival and departure times.

Indoor Space
At the time of the visit it was observed that the preschool playground had a play structure which had chipping and peeling paint on the slides, the handrails and the top of slide. The play structure had a "tunnel" that had unknown vegetation/growth covering the outside of the tunnel. There was not an adequate amount of woodchips at the base of the slides on the playground play structure. At the time of the visit the base of the slides had roughly 0.5-1 inch of woodchips. The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate.
At the time of the visit, in a review of three IHCPs and the corresponding prescribed medication, one student did not have their medication appropriately stored. One student had an antihistamine outside of its original packaging. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. Over-the-counter medications must be in the original manufacturer's packaging.
At the time of the visit, in a review of three IHCPs and the corresponding medication consent documentation, the information was not fully complete. One student was missing documentation of medication consent. Another student had two prescribed medications, both medication consent documents were expired. All medications must be accompanied by written parental consent and healthcare practitioner authorization when required.

At the time of the visit, in a review of three staff files, the documentation was not fully complete. Two educators were missing documentation of application/resume, verification of two references and an annual evaluation. Staff A was missing documentation of two MMRs, First Aid, CPR and current staff observations. Staff B had an expired PQR. Staff C was missing documentation of a staff form, and interview date. The program must maintain a personnel record for each staff member as outlined by the staff record checklist.
At the time of the visit and in a review of the document library, the program did not have a current Fire Inspection. The Licensee identified within email after the visit that the Fire Inspection was completed, but they were waiting on documentation from the Fire Department. Every program not located in a residence must submit evidence of compliance with applicable fire codes.
At the time of the visit, the following was observed: Within the Toddler Two classroom there was a bookshelf and a play kitchen unsecured and tippable. The changing table had plastic shopping bags stored next to the changing pad, accessible to students if they were to lay on the changing table. Within the Toddler Three classroom there was a larger toy crib that was unsecured and tippable. Within the Preschool classroom there was a play kitchen and a shelf/toy bin unsecure and tippable. The licensee must ensure that the physical facilities are safe, clean, in good repair and free from hazards and clutter. The licensee must monitor the environment daily to identify and remove or repair any hazards that may cause injury to children. The licensee must only use indoor and outdoor equipment, materials, furnishings, toys, and games that are appropriate to the ages, needs and developmental level of the children enrolled. They must be sturdy, safely constructed and installed and non-tippable.
At the time of the visit, in a review of the fire drill log, the documentation was not fully complete. All 2025 fire drill documentation was missing route used by the program. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
At the time of the visit, in a review of three IHCPs, the documentation was not fully complete. All three were missing documentation of educators trained on the health condition. Student A was missing documentation of medication dosage and did not have a parent signature on the IHCP. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.
At the time of the visit, in a review of three IHCPs and the corresponding prescribed medication, one student did not have their medication readily available. One student had their emergency medication stored in the front office. Emergency or life-saving medications, for any children for whom they have been prescribed must be readily available.

At the time of the visit, two refrigerators within the infant classrooms did not have thermometers. Thermometers verifying temperatures must be maintained in all refrigerators.
At the time of the visit, within the Toddler One classroom (I/T age group), three bottles were unlabeled within the refrigerator. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
At the time of the visit, in a review of three students IHCPs, two students doctor provided IHCP were missing a parent signature, one student was missing the EEC IHCP from their documentation. Another student had some of the EEC IHCP completed, but it was not fully complete.
This follow-up visit inspection recorded no violations or advisories.
Safe Sleep

During the visit, one 4 month old infant was observed sleeping in a swaddle, with her arms restrained. See attached Safe Sleep Policy, First Citation Instructions highlighted.
Chipped and peeling paint was observed in Toddler 1 Classroom.
There was not a current Individual Health Care Plan available for two children; Child A and Child N.

In Infant 1 the diaper changing pad was not secure; small green wall had chipped paint; white plaster board was chipped. In Toddler 1 a wall vent with visible dirt on it; Toddler 2 diaper changing mat did not fit the table therefore not sanitized, with visible dirt on the table In Preschool the door mat was a trip hazard, this was removed during the visit. In Preschool 2 a plastic tool bench was not secured. In PreK a cord to the radio wa draped in a manner accessible to children.
Some classroom exit doors did not have evacuations procedures/diagrams posted.
Some areas in classrooms were not clean, wth visible dust/dirt accessible to children. One rug in Toddler 2 was turned up at the corners, resulting in a potential trip hazard. During the walk through with the Provider, these areas were pointed out. One trash was not covered in Toddler/PS.
Infant car seats were lined up on the floor of the main hallway. This would potentially hinder a safe evacuation, in the event of an emergency. Alternatives were discussed with Provider during the visit.
