Individual Health Care Plan (IHCP)
At the time of the visit, in a review of three IHCPs and the corresponding Medication Consent documentation the information was not fully complete or current. Two students were each diagnosed with two different health conditions. One student had one expired Medication Consent documentation. The other medication consent documentation for the student was missing documentation on if the student had taken the medication before and the date for the medication to be provided. Four additional medication consent documents were KCE Champions created documents. All four did not indicate if the medication had been taken previously. The KCE Champions created documents were all missing EEC required information. All were missing the side effects of the medication, the directions for storage and the phone number of the prescribing physician.

At the time of the visit, in a review of three IHCPs, the documentation was not fully complete and was not current. Two students were each diagnosed with two different health conditions. Both students each had one expired IHCP for one (out of two) of their diagnosed health conditions. One of the students was missing documentation of the symptoms of their health condition, the medical treatment necessary, the side effects of the medication and the potential consequences if treatment is not administered. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.

At the time of the visit, in a review of three educators files, the documentation was not fully complete. All three educators were missing documentation of completing program orientation. All three had expired staff observations. The program must maintain a personnel record for each staff member. All educators must attend an orientation to early education and care approved by the Department. Staff observations must be completed bi-monthly.
At the time of the visit, in a review of three educators files, two educators did not have current documentation of completing a Background Record Check. In a later review of the BRC portal it was identified that both educators BRC's expired in March 2025. Evidence of completed background record checks must maintain in each personnel record. All candidates who are directly employed or intern within Programs must complete a full BRC through the Program, regardless of whether such individuals work in an unsupervised capacity.
At the time of the visit, it was observed that the cafeteria had four exits. All exits had evacuation routes posted, two were missing the procedures to be followed in an evacuation. Next to each exit, emergency and evacuation procedures must be posted.
At the time of the visit, in a review of three IHCPs and the corresponding medication prescribed, one student had two prescribed medications that were expired. Medications must be readily available and not be expired.

The program had two mini-refrigerators for students snack, neither refrigerator had a thermometer in it. Thermometers verifying temperatures must be maintained in all refrigerators.

At the time of the visit, in a review of four educators files, all four were missing documentation of bi-monthly observations, two were missing documentation of professional development hours, and two did not have an adequate amount of professional development hours for the previous year.
At the time of the visit, an educator was observed using a reusable wash cloth and a bucket of water with a cleaning substance it in to sanitize multiple tables prior to students eating lunch. A cleaning bottle in the library was not labeled with its contents and a plastic box that contained several cleaning solutions was located on a low shelf within the library. The disinfectant solution used to disinfect child care items, equipment and surfaces must be either a bleach solution prepared by the licensee in accordance with EEC guidelines or a commercially prepared disinfectant that has been registered as a sanitizing solution by the EPA.
There was not a current parental consent for administration of medication for one child with an (REDACTED).

A review of emergency evacuation drill log indicated that the effectiveness of the drill is not documented. (How long did the drill take)
Adult scissors (3 pair) were accessible to children. These were removed at the time of the visit.
This investigation inspection recorded no violations or advisories.
During the visit, children that were enrolled in the program did not have the required enrollment forms. The Program Director stated that they were in the process of collecting the enrollment forms from parents and that the children's records from their school year programs were available to staff.
Medication
A list of allergies, emergency medical information, and all emergency or life saving medications was not available at the program. Please submit.

At the time of the visit, the evacuation route diagram was posted but the evacuation procedures were missing. With the exception of the main exit out of the cafeteria, which had the required evacuation procedures posted.
At the time of the visit, in a review of three students mediations, one student had Benadryl that was not in the original manufacturer's packaging.
At the time of the visit a pump bottle of Hand Sanitizer was on a shelf accessible to students. The bottle had a manufacturers label that outlines to "keep out of reach of children".

CAFETERIA -Two garbage containers were not covered. BOYS AND GIRLS BATHROOMS -There was rust on the heater covers.
Six emergency medications being stored at the program did not have medication consent forms.
-Three prescription bottles containing (REDACTED) were being stored in a group's emergency medication and first aid backpack. -An expired (REDACTED) for a child attending the program was being stored with the current (REDACTED).
A review of four children's records indicated that four Individual Health Care Plans (IHCP) were incomplete.
