In the basement space, there were two electric fans on top of the tables accessible and tippable. In the bathroom in the basement, one drawer was not locked allowing access to gloves and screws.
In the outdoor play pace an unraveled hose was observed posing a tripping hazard.

Progress reports were not current for the enrolled children.
The Educator had children wash hands before eating lunch. She then put on gloves and changed a diaper. After changing the diaper and removing the gloves, she put the child in the booster seat and then began serving lunch to the children. She did not wash her hands or the child's hands after the diaper change and before they both handled food.
The records for five of the enrolled children were reviewed. All five needed Annual Updates and proof of current physicals, immunizations, and lead screenings.
This renewal - enhanced monitoring visit inspection recorded no violations or advisories.

The exit doors were not labeled in the basement.. Please send proof of labeled exits.
There was a great deal of clutter in the home on the of the visit (laundry piled on the sofa and chairs, drying racks with laundry, many shoes in the entry way, etc.)
The Educator did not have proof of current progress reports. She stated that she had moved her records due to having a birthday party recently and was unable to locate them at the time of the visit.

The four enrolled children’s records were reviewed, two children need an annual update form, two children need an updated physical and one child required a medication consent form and individual health care plan.
All four children enrolled need updated progress reports.
The educator did not have documented proof of an evacuation drill conducted since November 2021. The educator reported conducting them on a monthly basis but does not have them documented.
The educator did not have their parent handbook available for licensor tor review during the visit.
The educator has reported to have changed the swings utilized on the play structure which was grandfathered in. The educator will need to come into compliance with the fall zone requirement. As per policy any changes the educator makes to their existing play equipment must install fall zones.
The educator did not have required posting for one child with a medication allergy and one child with a food allergy with a life saving medication.
The educator did not have their ongoing professional development readily available for licensor to review during the visit.
The dining room was very cluttered with piles of items on the table that were tipping hazards. There was also a laundry drying rack set up with clothes on it, a bag with empty beverage cans, electrical cords hanging down from a keyboard, etc. accessible to the children in the dining room.
