At the time of the visit, in a review of three students IHCPs and the corresponding medications, one student was missing the proper measuring tool for their liquid medication. The student was prescribed "5mg" on their IHCP; the measuring tool on site did not have this measurement.
At the time of the visit, in a review of the outdoor play area, the woodchips surrounding the largest play structure's slide were roughly 0-0.5 inches in depth. The use zones under and around swings, slides, and climbing structures must be covered with an adequate depth of an impact absorbing material, in accordance with EEC policy.

At the time of the visit, the upstairs Pre-K classroom did not have evacuation procedures posted by the door leading outside to the playground. Next to each exit, please post emergency evacuation procedures.

Toxic Substances
One teacher purse was accessible to children, moved at the time of the visit. One heating vent on the bathroom wall had sharp knobs not covered, and accessible to children.

Staff orientation, training and meetings
Heating source protected; Portable heaters, portable radiators prohibited.
Baseboard heaters throughout the program did not have protective coverings, to prevent a child from touching the heating element.

Two children's records did not include Parental Consent for Administration of Medication, for prescribed medications.
This change of space visit inspection recorded no violations or advisories.
Indoor Space
The teacher in the downstairs room stated one child had a (REDACTED) but the medication would be kept upstairs if (REDACTED) had any medication.
Toxic Substances
The program maintained an attendance list at the front desk. This list included children from all rooms. The 3 individual rooms did not maintain (at all times) a list of children they were assigned to supervise at any given moment. In both upstairs rooms, the teacher counted children to determine the number of children in the room. In the middle room, one educator was assigned to supervise 12 preschool children. The teacher from the big upstairs room stated that 2 of the children were suppose to be in her room but the teacher from the Middle room had taken them for an activity. In the downstairs room, the educator stated that she had a sign in sheet but had not signed in any children today.
The downstairs teacher indicated she was unsure if a child with a (REDACTED) had medication. All educators must be aware of emergency medication used to treat chronic medical conditions.
Individual Health Care Plan (IHCP) - R Apr 2025
At the time of the visit, in the downstairs Preschool Classroom, one student was not signed into the attendance. The student was signed into the classroom at EEC's request. The program must maintain daily attendance records indicating each child's attendance, including arrival and departure times.
At the time of the visit, the students were eating off individual, laminated placemats instead of plates. When asked how the placemats were cleaned and sanitized in-between each use, an educator reported that baby wipes were used to clean the placemats. The licensee must ensure that equipment, materials, items or surfaces are washed with soap and water and disinfected to maintain a sanitary environment. All eating surfaces must be washed and disinfected before and after each use.
At the time of the visit, in a review of two students IHCP's one student did not have a complete EEC IHCP. The Medication Consent forms for both students were not fully complete and had information that was not outlined properly (such as dates and times the medication was to be given and directions for storage). Please fully complete all aspects of IHCP documents. One student had a medication consent form for a medication that was not outlined on their IHCP. Medication provided to students must correspond with the outlined IHCP or a note from the child's medical provider.
At the time of the visit, in a review of two students emergency/ life saving medications, one student was missing a second epi-pen when the action plan called for a second dose to be administered if needed. Another student was missing the measuring tool for their Benadryl.
Medication
Downstairs, the shelves and play kitchen were tippable. The educator stated that all the furniture was moved and had not been secured prior to children returning to the room.

Staff purses were hanging inside the door, which was accessible to children. The kitchen door (from the upstairs preschool room) was ajar. Inside was a stove, sharp knives and a toaster oven that were accessible to children. -In the Big room and downstairs room, hand sanitizer (marked keep out of reach of children) was accessible. -In the downstairs room, Method Cleaner and Windex were accessible to children in the kitchenette. In addition, there was clutter and unsecured furniture in the kitchenette. Also downstairs another room was accessible which contained paint/ stain and an unsecured shelf.
