The attendance records did not accurately reflect the times of arrival and departure for the children. All times of arrival were the same and all times of departure were the same. Some of the children are school-aged, so there should be two times of arrival and departure for these children for both before an after school.
The evacuation log form was pre-filled out on one form for entire year. There was another form being used that was not the correct form and did not have all the required information.
The educator did not have proof of the required 30 hours of professional development training hours. Please send proof of 6 more hours.
The fencing in the outdoor space was broken, allowing a large gap in the fencing and access to the in ground pool.
