Playground Safety and Ground Surface
During the visit, the web- shaped climbing structure at the front of the playground had exposed wires where children grip with their hands to climb. There is a coating of plastic over the wires but the wires are exposed in multiple spots, causing a potential hazard to children. 7.07(07) Article The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate.

During the visit, the mulch surrounding the climbing structures was approximately four inches in depth. The use zones under and around swings, slides, and climbing structures (measures six feet from the bottom of the equipment) must be covered with an adequate depth of an impact absorbing material, in accordance with EEC policy. Pea gravel and wood chip nuggets must not be used in areas used by infants and toddlers.
The evacuation procedures were not posted at the exterior of the door at the back of the cafeteria.
One child's IHCP was not completed. The parent had listed multiple conditions and did not include the treatment, potential side effects of the treatment, the consequences if treatment is not provided and who at the program can administer the treatment. Each chronic medical condition needs to have its own IHCP filled out.
