The following was observed during the visit: In INFANT/TODDLER, the plastic and metal stacking chairs each had areas of rust on the metal arm rests/structure. The licensee must only use indoor and outdoor equipment, materials, furnishings, toys, and games that are appropriate to the ages, needs and developmental level of the children enrolled. They must be sturdy, safely constructed and installed, non-tippable, flame retardant, easily cleaned, and free from lead paint, protruding nails, rust, and other hazards that may be dangerous to children.
The following was observed during the visit: The changing pad in the TODDLER/PRESCHOOL classroom had a tear at the corner. The changing surface must be smooth, intact, impervious to water and easily cleaned.

The following was observed to be accessible to children: Approximately 12 small rocks were found on turf ground surface in the Infant/Toddler outdoor space and are a choking hazards. The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate.

The following was observed to be accessible to children: -In the PRESCHOOL/BIG TODS area, pieces of garden cloth were emerging from the mulch and presented as a tripping hazard. -Two slides on separate climbers were facing one another and the exits of the slides were with approximately 11 feet of one another. One of the slides (attached to the truck) was moved during the visit to create the twelve feet of space to come into compliance.
Current Building and Fire Inspections were not available during the visit (expired 2/1/2023). Director reported the program was inspected on 9/5/2023 and they are awaiting certificates.
A review of the Staff Meeting Binder was conducted and found that staff meetings were missed: In the last year, staff meetings were held 8/26/2022, 11/2022, 1/5/2023, 3/2023 and 5/16/2023. In programs with four or more staff the licensee must develop and follow a written plan and must document regular staff meetings of at least two hours per month to consult with educators on program issues; program planning; policies and procedures; parent communication; implementation of 606 CMR 7.00; children’s behaviors and meeting the individual needs of children.

A review of the Emergency Drill Log indicated drills have not been done monthly as required. The log has not been current for the past two years.
Two staff records were reviewed. The first staff record did not have evidence that two references were conducted, did not have evidence of completion of the StrongStart trainings, nor did they have evidence of registering with PQ. The second staff record did not have a physical examination within the last two years, the CPR/FA expired 2/2022, the PQ expired 9/7/2020, and the record did not have evidence of reference checks.
The TODDLER ROOM attendance indicated six children were present, however, eight children were present. This attendance sheet was observed at approximately 10:30am.

The following hazards were accessible to children: PRESCHOOL ROOM plastic bags and glade scent spray in an unlocked drawer. BIG TODDLER ROOM plastic bags and q tips, plastic bags, 12 tubes of diaper cream, plastic hair bands, a screw, pepto bismol medication were accessible to children in unlocked drawers. Two first aid backpacks were accessible to children on the playground,.
Equipment and supplies: free of entrapment hazards
One (REDACTED) was observed sleeping in a crib, on his tummy, wearing a hooded jacket. The hood was on his head. One 9 month old, and one (REDACTED) were obsrveds sleeping in pac and plays with sheets that did not fit the mattress properly, resulting in excess fabric accessible to (REDACTED). One sheet was bunched down exposing a large section of the bare mattress. In addition, leaning on the outside of these pac and plays were nap mats, which could prevent air flow through the sleeping area.

Cords more than 6 inches were accessible to children in the PRESCHOOL ROOM.
The changing table in Toddler/Preschool did not have a protective covering available for educator's to use on top of the changing surface. In programs serving children who are under two years and nine months of age and/or not toilet trained, the educator must ensure that: the changing surface is protected with a covering that is of adequate size to prevent the child from coming in contact with the changing surface.
The following was found to be accessible to children: -In INFANT/TODDLER, the wooden kitchen set was not secure and tippable. -In TODDLER/PRESCHOOL, the trash bin was missing a cover. -In PRESCHOOL/SCHOOL AGE, the tool bench and plastic kitchen set were not secure and tippable.
The Fire Drill Log was observed and found monthly drills have not been conducted since May 2024.
The IHCP for a child with asthma was incomplete. It was missing the names of educators trained to administer the medication.
Three child records were reviewed and found: -One child's record was missing the physical and did not have an IHCP although the parent wrote the child (REDACTED).

Three staff records were reviewed and found: -One staff record (hired March 2023) did not have any bi-monthly observations. -One staff record (hired 2021) was missing many bi-monthly observations (only had June 2023) and was missing annual evaluations.
The following was observed to be accessible to children: -In INFANT/TODDLER, the bathroom contained a toilet brush and plunger. -In TODDLER/PRESCHOOL, wipes in plastic bags were under the changing table (moved during visit); Desitin and one loose glove were found in the bathroom; the cabinet under the sink was unlocked (lock was broken) and contained Lysol and a powder cleaning product that state Keep out of reach of children. -In PRESCHOOL, one cubby system was tippable and not secure.
Paint was observed to be chipping in the INFANT/TODDLER classroom along the right hand wall in the table area, next to the caterpillar wall hanging, and next to the alphabet posters.
In INFANT/TODDLERS, an educator was observed to change a child's diaper and did not wash the child's hands afterward.
The sink in the INFANT classroom used for handwashing contained a teether and a small toy rake indicating the sink is used for handwashing and rinsing/cleaning items.

As indicated in Staff Records Review, one staff member's First Aid Certification expired in 2/2022.
Three child records were reviewed. The first child record did not have a current developmental history. The second child record did not have a current developmental history or current physical examination. The third child who just began at the program is missing the physical examination and immunizations. 7.08(5)(3) states, "The developmental history must be updated annually and maintained in the child’s record."
The staff schedule for the infant and toddlers rooms were not posted during the time of the visit.
The second crib from the corner in the NFANT ROOM had a loose sheet on the mattress, as indicated by four wrinkles in the middle. The crib was empty. 7.11 (13)(e)(3) 3. Ensure that cribs have firm, properly fitted mattresses with clean coverings, and do not contain any potential head entrapment areas.
Peeling paint was accessible to children on one wall in the TODDLER ROOM.
In the bathroom off the BIG TODDLER ROOM Clorox and an Air Freshener both labeled "Keep out of reach of children" were accessible. Lysol wipes were also accessible to children in this classroom under the cabinet in the snack/art area.
A (REDACTED) was observed seated in a high chair; she was not buckled in. Staff stated that there were no straps on the high chair, as provided by manufacturer.

The tarp draped over the sand box held approximately 4-5 inches of water.
The diaper changing pad in Infant Room had numerous creases, preventing a thorough sanitizing.
