Two staff records were reviewed and it was found that one staff record did not have a current physical.
The refrigerator in the PS/SA classroom did not have a thermometer. The licensee must provide refrigeration and storage for food at not less than 32 degrees F, nor more than 40 degrees F for all food requiring refrigeration. Meals and snacks provided by parents must be stored safely. Thermometers verifying temperatures must be maintained in all refrigerators.
The following was observed to be accessible to children: Two white shelving units with the gray cushions on top were not secure and tippable.
Equipment is clean, safe, secured, and poses no hazard.

The fencing surrounding the stairwell in the green/sun room is an entrapment hazard on two sides (short side with the gate and the backside facing the outer wall). The two additional sides are covered by furniture. The fence rungs are approximately 3.7 inches apart. An entrapment hazard is defined at "Any opening that is greater than 3 1/2 inches but less than 9 inches that can entrap a child’s head creating risk for serious injury or death." All play equipment, fences and structures must be free of entrapment hazards.
The green/sun room does not have a sink with running water adjacent to the diapering area. There must be running water or an approved alternative adjacent to each diapering area.

Licensors observed the Preschool group remain outside after the evacuation drill was conducted and observed the Preschool educator latch the lock on the fence gate to the side porch and cover the grill while the children sat at the porch table or wandered about the porch. The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate.

Licensors asked (REDACTED) to conduct an evacuation drill log "the way you would normally conduct a drill". The drill began at 10:04am and finished at 10:13am when the educators and children stopped at the shed in the back of the yard. After approximately ten minutes, Licensor AH asked Director is she'd documented the drill and the Director said she had not observed the time because "there is so much going on". Licensor gave Director the beginning and ending time of the drill. The evacuation drill took a substantial amount of time because both groups separately had to get the children's' shoes on them and the Preschool group was not informed they were having a fire drill until three minutes after the Toddler/Preschool group began their evacuation process.
Three child records were reviewed (emailed to Licensor on 8/16/2024). -One child record was missing a physical and lead test. -One child record did not have a current child enrollment form, developmental history or transportation plan (all expired 5/19/2024). -One child record did not have a current child enrollment form or developmental history (expired 7/31/2023).
The current staff schedule was not posted. The Licensee must keep the schedule current and posted in an area easily visible to educators, parents and visitors.

Children and staff in the two mixed groupings were observed to be using all licensed space as there was not a clear identification of classrooms. Each child must be assigned to a consistent group with consistent staffing.

Entrapment Hazard
The Plan for Diapering was not posted in the green/sun room. A written plan for diapering and toilet training and for the disposal or cleaning of soiled clothing, linen, blankets and diapers must be developed, implemented and posted in diapering areas.
The white shelving unit in the classroom immediately on the left as you enter the classroom is not secure and tippable. The licensee must keep all equipment, materials, furnishings, toys, and games clean and in a safe, secure, and workable condition.
The green/sunroom did not have evacuation procedures posted that include how they will meet the needs of all children in care, including infants, toddlers and any children (including but not limited to those with disabilities) who may need additional assistance during an evacuation. The licensee must post the following information: 4. next to each exit, emergency and evacuation procedures.

The following was observed by licensors during the visit: The Lead Teacher of the Preschool group left the nine preschool children alone in their classroom to answer the door when licensors arrived. The Director was not in the EEC program licensed space when licensors arrived. Director was observed walking from the private residence to the program via the hallway that connects the two spaces and was observed to walk through the gate and into the program space. The Director was assigned to the Toddler/Preschool grouping at the time of the visit. The second educator in the Toddler/Preschool group was alone with six children (ratio is 1:5) when Director was out of the program space/classroom. The Director stated she was throwing away a dirty diaper in the trash located in the residence. The Director and second educator in the Toddler/Preschool group also each left the classroom to retrieve shoes for the children in the vestibule during the fire drill. They each closed the door separating the classroom and the vestibule behind them. A conversation about the sunroom was had between the licensors and the Director. The Director stated the sunroom would be an "overflow room" and would be used for nap time or physical activity such as yoga when the children are not able to go outside due to the weather. Licensors explained that ratios must always be maintained if using this room and an assistant teacher would not be able to be alone with children in the sunroom as this is viewed by EEC as a separate space/room.

One of the staff records reviewed had an online certificate for a CPR/First Aid training. EEC does not accept online only training. EEC Policy- Online CPR training may be taken as long as the participant is required as part of the online training to demonstrate and successfully complete age-appropriate CPR techniques to a certified instructor. EEC will not accept CPR certification training that does not include hands-on skill demonstration. Online first aid training may be taken as long as the participant is required, as part of the online training, to demonstrate and successfully complete first aid techniques to a certified instructor. EEC will not accept first aid certification training that does not include hands-on skill demonstration.
After reviewing CPR/First Aid training certificates it was found that none of the staff have current CPR certification. Two staff members cards expired 1/9/2024, one staff is new, and one completed an online training that is not accepted by EEC.
Two staff records were reviewed (emailed to Licensor on 8/16/2024) and found: -One staff record was missing evidence of completion of EEC Essentials 2.0. -One staff record was missing evidence of completion of EEC Essentials 1.0, evidence of professional development, and evidence of MMR vaccination.
The Building and Fire Inspections are not current. The Building Inspection expired 6/30/2024 and the Fire Inspection expired 6/29/2024.

A master attendance was viewed but the program did not have two separate attendances for the two groupings identified on the current license. The program must maintain accurate attendance, staff child ratio, and appropriate group size.
The following was observed to be accessible to children: -In CLASSROOM 1 space, the gate leading to the residence was open. The gate was closed when the licensor brought it to the director's attention; The gate between the entry area of the eating area was loose and was observed to swing when a child leaned on it. This gate would become a hazard if it gets loose and falls; The 4-unit cubby system was not secure and tippable. -In CLASSROOM 2, the purple shelving unit, three cubby systems with four units, one cubby system with five units, and one cubby system with eight units were not secure and tippable; two holes in the wall near the chalkboard had exposed plaster; and two window shade cords hung loosely from the hooks they were tied around which are a strangulation hazard.
The following was observed to be accessible to children: -Black garden cloth was visible in one of the garden areas and is a tripping hazard. -A lawn mower and gas can is in the unlocked shed in the backyard. -A box of open gloves and loose electronic wires were found in the drawer of the black cabinet sitting on top of the deck. -A stand of lights hanging on the deck is hanging low and the cord was stretched across the ground near the outlet. Both are a strangulation hazard.
The director stated the sunroom is used for a napping area for the toddler/preschool grouping with one staff supervising, however, this space has not been approved by the Department. Director stated this space has been added to the license renewal transaction.
