Records were reviewed for six of the twelve enrolled children. The children who have been enrolled longer than one year needed Annual Update forms. The General Permission section of the enrollment packets was not consistently filled out to reflect walks around the neighborhood.

The educator did not have proof of enough professional development training hours. She did have the certificate for the EEC Essentials 2.0 trainings. Please send proof of 18 hours of training, including the required diverse learners trainings.

Educator is documenting daily attendance for all children. Educator has not been documenting attendance for her assistant when she is present and functioning as her assistant to include when assistant is alone with children the educator must ensure attendance logs reflect accurately who is present
Review of educator’s attendance records reflects the educator being out of her licensed capacity on a regular basis for the past 2 months due to her own infant counting into her capacity even though infant is not in daycare space. Capacity on multiple days reflects a total of 9 children present due to her own infant. It was additionally observed that on one day during the month of June 2022 there were 10 children present, which includes educator’s infant. The educator has been cited before for being out of her licensed capacity, this was observed during a visit in 2021 and 2016.

Based on the review of the educator's daily attendance records, it reflected that the educator was out of her licensed capacity during one day in February when there were 9 children present. The educator is only licensed for up to 8 children of which at least two must be school aged.
Based on the educator's daily attendance records; it reflected that the educator has been out of her ratios on various days. The educator has six children under the age of 2 years old currently enrolled, which includes 3 infants. It was observed that on at least four days in the month of March and the month of February the educator had four children under the age of 2 years old present at the same time. There were also two days in which the educator had 8 children present and only one child was school aged.

Educator’s daily attendance reflects educator being out of her licensed ratios on a regular basis for the past 2 months due to her own infant counting into her ratios even though infant is not in daycare space. Ratios reflects up to 4 children under the age of 2 years old, which includes her own infant and 7 children present on multiple days which does not reflect school aged children present. This again is due to her own infant counting into her ratios. Educator has been cited before for being out of her licensed ratios during a visit in 2021 and 2016.

Evacuation drills were not documented for months of March, April, and May 2022. During today’s visit licensor observed educator and children practice their fire drill.
