Educator completed online CPR and First Aid renewal on 1/2022. Online only renewal is no longer valid. Educator must completed in person CPR and First Aid renewal by 9/24/2022 and submit proof.
Children did not have up to date progress reports. Licensor will send educator copy of an assessment tool.

Two children’s times of arrival were not documented for the date of the visit. A review of the attendance records showed some other days when times of arrival and/or departure were not documented (October 1 and October 18, 2024)
There were 6 children with the educator on the day of the visit. Four children were under the age of two years old. Three of those children were (REDACTED). There are two other children enrolled are also under 2 years, but were not present on the day of the visit. A review of the attendance records showed the educator has been out of ratio since the beginning of September.

The educator did not wash her hands or the children’s hands when the children came inside from the outdoor play space and before having snack. She did wash hands after the children ate snack.

There was a child sleeping on the second floor level of the home. The Educator was not on the same floor level as the child.

Records were reviewed for the 11 enrolled children. One child had a blank General Permission section. Two children had an incomplete enrollment packet. One child was missing the signature for the Acknowledgment of Receipt of the Parent Handbook and proof of current physical, immunizations, and lead screening. Three children were missing proof of current medical documentation.
There was one child with documented allergies. There was no allergy list.
The outlets were not covered in the licensed space. There was no gate for the stairs leading up to the second floor.
There was a child sleeping on the second floor. None of the rooms on the second floor are approved as licensed space.
The carbon monoxide detector on the first floor was not working and the one on the basement level was missing.
