The allergy/chronic conditions lists posted in the classrooms were observed and were missing one child's name and their chronic medical condition and medication. In a manner that protects the privacy of the children, the licensee must post a list of all emergency or life saving medications, including but not limited to epinephrine auto-injectors, inhalers, and anti-seizure medications, that specifies to which children they belong.
The following areas were observed during the visit to have peeling or flaking paint: In PREK, yellow paint is peeling in two spots on the left of the stage, under the bulletin board and the window ledge on the window to the right of the exterior door has peeling paint, and the blue paint is peeling in a few spots on the interior wall in the block area. In PS1, paint was observed to be chipping along the outer purple wall. The licensee must maintain the interior and exterior of the program in good repair, free of chipping, flaking, or peeling paint or broken plaster.

Staff Meeting documentation was reviewed during the visit and found evidence was missing for a meeting held in October 2024.
Five child records were reviewed and found one record is missing evidence of a lead screen.
The program does not have a current Health Care Consultant Agreement signed.
The following was observed to be accessible to children during the visit: Unknown vegetation is growing through the outer fence along the parking lot, the sections of fencing where the toddler and preschool areas connect are missing brackets to hold the fence in place next to the poles, which could lead to children squeezing through, and in the preschool area, the closed double gate leading to the parking lot has a wide space where the two gates close together approximately 6 inches where children could slip through the center and leave the space. The outdoor play area must be free from hazards including but not limited to: a busy street, a parking lot, poisonous plants, water hazards, debris, broken glass, chipping, peeling or flaking paint, dangerous machinery or tools, and weather-related and environmental hazards or small objects that could present a choking hazard to young children. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate.
An antihistamine required for emergency treatment was not available for a child during the visit.
An IHCP is missing for a child with a chronic diagnosed medical condition. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.
The Building Inspection is not current (expired 7/24/2025). The Fire Inspection is current and expires 12/13/2025. The IPMs are current and expire 8/12/2026.
A consent form giving consent for staff to administer an antihistamine was missing.
Medication
Three children's water bottles in PRESCHOOL 2 did not have labels.
Although all educators were knowledgeable about the children's medical needs, none of the IHCPs had the following required information: who is trained to administer the medication and who trained them.
The Building Inspection is current and expires 7/24/2025 but the Fire Inspection is not current (expired 7/19/2024).

Children's belongings, storage space
In INFANT/TODDLER, the sheets were observed to be on the sleep cots and the cots stacked on top of one another. The linens were observed to be touching and the feet of the cots were touching the sheets on the cot below. The program must provide sleeping materials that are individually marked and in good repair and clean; and ensure safe and sanitary storage of blankets and bed linens.

The following were observed: -Broken flooring around the drain in toddler 1. -A vinyl chair was torn, exposing the padding inside. -There was chipped paint on a wall outside the children's bathroom.
The following were observed outdoors: -There was insufficient mulch in one area (out of 3) that was measured. -There were berries on a bush, that protruded into the playground. -There was rust on one cart and on the fence. -There was unidentified vegetation coming through the fence in the play area that could be used by infants and toddlers.

The books in the Infant 2 room were not age appropriate for infants. The books were paper and were visibly crumpled and bent.

A review of 3 staff files indicated: -observations were not consistently individualized. Observations in files sometimes included the names of other staff and evidence of consultation with the other staff documented in the observation.
The following were observed: -Infant 2: The pillow in the swing was soiled. -Hallway door: The plaster around the door to the outside was cracked and exposed.

The following were observed: -Gloves and a plastic bag were accessible to children in the bathroom area outside of the Toddler 2 room. -A laminator was accessible to children (sitting on the handwashing sink) in PS 2. The laminator was warm to the touch. The box states that this should not be accessible to children.

The following was observed to be accessible to children: - In TODDLER/PRESCHOOL, a long black extension cord near the radio was loosely hanging from a bookshelf and was approximately three feet in length. Strings and cords longer than six inches that are not part of recreational or educational materials, including, but not limited to cords on window blinds, curtains or shades, must be kept out of children’s reach. The Director shortened the cord during the visit. -In PREK, a screwdriver was sitting in a pencil cup on a low shelf near the windows. The screwdriver was relocated out of reach of children during the visit. -In PRESCHOOL I, plaster was showing on the bottoms half of the purple wall near the windows and some paint was peeling next to the window on the right.
The following was observed to be accessible to children: The large toy tool bench was against the fence but was not secure and tippable.
The emergency backpacks containing emergency medication for children in PRESCHOOL I and PRESCHOOL II were observed to be in reach of children. The bag for PS I was sitting on the ground next to the picnic table and the bag for PS 2 was sitting no the bench of the picnic table. Both bags were relocated to be out of reach (hung on the fence) during the visit.
One child's antihistamine and another child's EpiPen were not available at the program.
The following was observed to be accessible to children: -In TODDLER/PRESCHOOL, an outlet cover was broken and had a sharp edge. -In TODDLER 2, the hop scotch area rug was curling at one corner and presents as a tripping hazard. -In INFANTS, a hole was found in the wall next to the outside exit door and plaster could be seen.
The following was observed to be accessible to children: -In TODDLER/PRESCHOOL, one outlet was uncovered at the far end of the wall. -In TODDLER 2, one outlet was uncovered near the Preschool 2 room.
The following was observed to be accessible to children: - In the TODDLER 2 BATHROOM, the plastic drawers contained packages of baby wipes in plastic bags. These are considered a suffocation hazard. -In the TODDLER 1 BATHROOM, plastic drawers contained skin ointments, sunscreen, and baby wipes in plastic bags. The ointments and sun screen state on the labels to "Keep out of reach of children".
One big composite structure and two smaller slides did not have the required depth of impact absorbing material. In addition neither slide had an adequate use zone.
