The Staff Meetings were reviewed and found staff meetings held were only 1 hour in duration. In programs with four or more staff the licensee must develop and follow a written plan and must document regular staff meetings of at least two hours per month to consult with educators on program issues; program planning; policies and procedures; parent communication; implementation of 606 CMR 7.00; children’s behaviors and meeting the individual needs of children.

Four child records were reviewed and it was found: One child's record did not have a medication consent for a child's emergency medication.
One child's IHCP was incomplete. The side effects of treatment and consequences if treatment is not provided were missing.

The attendance sheet was observed during this visit and found two children had not been signed out when they departed the program.
The evacuation drill log was observed and found: -A drill was not conducted in October 2023.
A review of a physical examination for one child stated that the child would need (REDACTED), in the event of an emergency reaction. There was not parent supplied (REDACTED)available at the program for this child.
Emergency medications readily available
A review of four educator files indicated that three educators did not have current first aid training.

Rust was observed in the Boys and Girls bathrooms on the radiator covers.
-A review of medications indicated that an Epipen stored at the program was not stored in the original packaging. All prescription medications must be in the containers in which they were originally dispensed and with their original labels affixed. -During the visit, the licensor observed the backpack that stored emergency medications was on the stage accessible to children. This was corrected during the visit.
Food is nutritious, adequate amount, with variety/meets USDA requirements

There were two ceiling tiles in the music room that were stained and appeared to be wet. This is a sign there may have been a roof leak.

A projected one-year operating budget that estimated income and expenses was not available at the program for review. Please submit.
A review of four staff files indicated: -Two files did not have evidence of a current physical exam. -One file did not have a completed Staff Information Form. -One file did not have evidence that they participated in a program orientation. - Four of the files did not include bi-monthly staff observations. Staff stated that bi-monthly staff observations had not been completed in the last year.
On the day of the visit, there was no completed attendance available for review. At the licensor's request, the attendance was completed. The licensee must maintain daily attendance records indicating each child’s attendance, including arrival and departure times.
The current building inspection certificate and fire inspection certificate was not available on the day of the visit. The building inspection at the program expired on 9/29/21.
A review of two children’s records indicated two Individual Health Care Plans (IHCP) were incomplete.
On the day of the visit, there was no weekly snack menu available for review. Previous menus were not maintained in writing for one year.
The snack today consisted of Smartfood popcorn and water. USDA requirements for snack were not met.
