Current consent for administered medications
The following was observed during the visit and accessible to children: In TODDLER 1, paint is peeling under the interior window looking into PS classroom.

One staff record (new staff member) was reviewed today and found it was missing evidence of a physical and that an interview was conducted.
The posting for chronic conditions of children was not complete. Two medications were missing for two children with allergies and one chronic condition and medication for another child was missing. A list of all emergency or life saving medications, including but not limited to epinephrine auto-injectors, inhalers, and anti-seizure medications, that specifies to which children they belong.

The two staff records reviewed did not have evidence that they are currently certified in First Aid.
The following was observed to be accessible to children: -Toilet brushes were sitting next to each of the toilets in the bathrooms.
The following was observed: -In the PRESCHOOL space, two of the three slides did not have sufficient amount of mulch in the fall zones (six feet around the bottom of the slide), the steps on the larger structure were not a minimum of 6 feet away from the concrete, and the kitchen set was not secure and tippable.
IHCPs were reviewed for children with chronic health conditions and found: -Two IHCPs were not complete. One was missing who is trained to administer the medication and one was missing the potential side effects of the treatment.
