Progress reports were not current for the enrolled children.
There were 8 drawers in the kitchen and 4 drawers in the bathroom that had hazards in them (sharp knives, scissors, electrical cords, serrated edges from wraps and foils, personal care products, plastic bags, etc) that were accessible to children. There were two cabinets in the kitchen that had hazards in them (cleaning products, plastic bags, etc.). There was a vacuum cleaner in the living room that is a tipping hazard and a broom/dustpan in the dining room that was also a tipping hazard. The bathroom toilet was in need of cleaning.

Educator has a family member visiting for a few days. Visitor was observed interacting with the children. It was discussed with the educator that visitor does not have a background record check and interactions with children needs to be limited or a background record check needs to be submitted.
Children’s records were reviewed. Multiple files had incomplete enrollment packets, no annual updates, and no updated or complete physicals. Educator will need to complete and submit the children’s records checklist.
All children enrolled are due for updated progress reports. Last progress reports were completed in October 2021.

It was observed on the educator's attendance records that there were three Fridays, two in October and one in September where some children were not signed out.
This follow-up visit inspection recorded no violations or advisories.
There were seven children present. The educator is only licensed for a capacity of up to 6 children. The educator stated her(REDACTED) is visiting (REDACTED). The educator stated her (REDACTED) here for the holidays but not staying in her home, (REDACTED) staying with other family. It was reviewed with the educator that her (REDACTED) who were present today, during daycare hours, count into her capacity. This places the educator out of her licensed capacity by having seven children. Capacity and ratios have been regulations that have been reviewed with this educator multiple times before during prior visits. An additional daycare child arrived to the program about 25 minutes into the visit, which brought the total of children present to 8.

There was a bush with large, sharp thorns near the deck stairs that was accessible to chidren.

Records were reviewed for the eight enrolled children. Six of the children needed Annual Update forms. Three children needed proof of current medical documentation (physical, immunizations, and lead screening.)
The educator did not have proof of the required 30 hours of professional development training. She had 10 hours of training from the Food Program, but did not have proof of any other hours. Please send proof of 20 hours of professional development training, including 10 hours of diverse learners trainings.
The educator did not have proof of current progress reports on file for the children. The educator reported that she gives progress reports to the parents. It has been previously discussed with the educator that progress reports must remain in the children’s files.
The educator did not wash her hands or the child’s hands after changing a diaper. She also did not wash her hands or the children’s hands before eating.
The stairs to the second floor were accessible to children. This has been discussed with the educator previously. Also, discussed there needs to be a gate at the bottom of the deck stairs when children under 3 years old are playing outside.
There were 6 drawers in the kitchen that had hazards in them (knives, scissors, cords longer than 6 inches, etc). Some drawers had safety latches, but they were not installed properly, allowing access to the hazards in the drawers. There were drawers in the bathroom that had hazards in them, like ointments, spray perfume, etc. One drawer had a safety latch, but the latch was not working. The trash in the bathroom did not have a cover.
There were a total of seven children present, which includes the educator’s (REDACTED), who are visiting. There were no school aged children present. The educator’s assistant was present (REDACTED)but not functioning as an assistant. The educator’s (REDACTED) were downstairs with their father and grandmother, which is the educator’s assistant. Sixth daycare child arrived about 25 minutes into the visit, this brought the total children present to 8. Ages of children present were; (REDACTED)
Attendance records for today did not reflect the educator’s total of number of children present, (REDACTED) count in her capacity. The educator did not have prior attendance records available. The educator has been cited for not having accurate attendance records during prior visits.

It was observed that there was no gate on the stairs. When children under 3 years old are present, access to stairs must be presented. There was access to two vacuum cleaners and a broom. Access to cleaning materials must be prevented. It was observed that the children’s coats and jackets were stacked one on top of the other, and not stored in a sanitary and individual manner as requerid.
