A review of all children’s emergency medications was conducted and found three children were missing EpiPens (two children only had one EpiPen and they need a second, and the third child had an expired EpiPen so they need 2 current ones).
Two medication consent forms for a child were not current (expired 10/30/2025).
Last calendar year’s evacuation drill log was reviewed and found it was complete, however, this calendar year’s log had not yet been completed and the Director reported a drill was not conducted in April 2026 due to the weather.

The following was observed to be accessible to children in the outdoor space: In the Preschool area, the wood and metal truck had a lot of red chipped paint and the fencing adjacent to the green building at the back of the area, was loose at the bottom and can be lifted up (a hazard if children were to crawl underneath).

Electrical Outlets
The following was observed during the visit: In ROOM 1, the electrical outlets were not covered. All electrical outlets within the reach of children younger than school age must be made inaccessible by use of a safety device or covering that prevents access to the receptacle openings. If the covering is a shock stop, it must be of adequate size to prevent a choking hazard. All electrical cords must be arranged so they are not a hazard to children. Electrical cords must not be frayed or damaged.
The bathrooms do not all have Diapering and Toileting Procedures posted. A written plan for diapering and toilet training and for the disposal or cleaning of soiled clothing, linen, blankets and diapers must be developed, implemented and posted in diapering areas.
Two children's antihistamines were not on site at the time of the visit. One antihistamine was provided for one of these children but it was not the type of antihistamine the doctor noted on the action plan.

The list of children's chronic medical conditions and medications was not current in the Preschool Classroom. It was missing one child's name with condition and medication.
The following was observed to be accessible to children: In the PRESCHOOL Classroom, a large spot of missing/peeled paint was found on the right side of the classroom under the middle window. The facility must be free of chipping, flaking and peeling paint.

In TODDLER, two children's water bottles were not labeled with their names. In PREK, most of the children's water bottles were not labeled with the children's names. Personal items intended for individual use by children, including but not limited tobottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
Two children with allergies did not have antihistamine available as needed. Another child did not have antihistamine an EpiPen and the antihistamine (was in the director's office) available as needed.
All children with chronic health conditions were missing IHCPs.
The exit of the blue slide in the toddler area facing towards the other building was within four feet of a tree. There must be a clearance of at least six feet from the bottom.

Three staff records were reviewed and found: -Two staff files were missing annual evaluations and have not had bi-monthly observations since January 2023. -One staff file was missing the job application.
The following was observed to be accessible to children: -Paint was chipping under the windows in PRESCHOOL MOUSE and the TODDLER classrooms on the walls under the windows.
At the time of the visit the indoor and outdoor pool inspections and the indoor pool roof inspection were not current.

Toxic Substances
-A child's (REDACTED) prescription expired on (REDACTED). -A pack of (REDACTED) tablets were not stored at the program in their original packaging. In addition, the child's required dose was (REDACTED) and the (REDACTED) tablets were (REDACTED). The educator's did not have a way to administer the correct dose of medication to the child in an emergency situation.

Five classrooms and one indoor motor skill space were observed during the visit. The following was observed to be accessible to children: in ROOM 2, a large area of paint was chipping along the outer wall under one of the windows and the furniture (3 bookshelves) were not secure and tippable; The indoor space must be clean, safely maintained, well-ventilated and well-lit, of sufficient size for the children served, and must encourage play and learning.
During the visit, the following was not yet posting: 1. the location of the health care policy; 2. the name, address and telephone number of the health care consultant; 3. the location of the first aid kit; and 4. next to each exit, emergency and evacuation procedures. In addition to the information required to be posted by 606 CMR 7.04(14) the licensee must post this following information within the program space.
Three staff records were reviewed and found: One staff record was missing an annual evaluation from 2024 (had previous evaluations on 2/9/2023 and 2/5/2025) and a bi-monthly observation was missing from November 2024. One staff record was missing one bi-monthly observation (last one dated 3/12/2025). One staff record did not have a current PQ registration (expired 3/19/2025).
The following was observed to be accessible to children: the large ride on metal and wood truck has some red paint chipping.

Attendance was current in all groups but when two groups were outside, they combined. Maximum group sizes must be maintained at all times, even outside. Groups can occupy the same space but should be separated into different areas so supervision by the assigned educators can be maintained.
The indoor and outdoor Pool Inspections are current and expire 5/31/2026. The Pool Roof inspection is not current (expired January 2025). The Pool Roof Inspection Policy was emailed to director on 6/18/2025 and the request to obtain a supplemental document was requested.

When Licensor arrived to the visit, staff stated they were not sure who the designated person in charge was at the time. The Director had the day off.
Two staff records were reviewed and found: -One staff record was missing bi-monthly observations (hired in 3/2023).

The following was observed to be accessible to children: -A free standing basketball hoop was not secure and tippable.
A review of the staff records indicated that one staff did not have evidence of a current physical examination. The name of the educator was shared with the Program Director in the exit interview.
The following was observed and accessible to children: PRESCHOOL -A staff's personal bag and purse was stored on a table in reach of children. -A CD player cord longer than six inches. -A plunger behind the toilet in the bathroom. TODDLER -A heater grate behind the toilet in the bathroom had rust and dust on it. -The back wall in the classroom had chipping and peeling paint. PRE-KINDERGARTEN -A heater grate behind the toilet in the bathroom had rust and dust on it. -A charging port and CD player cords longer than six inches. -A play kitchen and two play kitchen storage units were tippable.
The exterior of the building had numerous areas of chipping and peeling paint. The stairs that lead to the entrance of the building also had chipping and peeling paint.
A review of the program's Individual Health Care Plans (IHCP) indicated that one child did not have their plan renewed annually as required. The plan expired on 6/22/2022.
During the visit, the kitchen and janitor closet were both unlocked. The kitchen had numerous hazards accessible to children including the stove and approximately ten cleaning solutions that were being stored under the sink. The janitor's closet had cleaning supplies and equipment being stored inside.
