Current consent for administered medications
Two staff records were reviewed and found: One staff record was missing evidence of immunity to MMR and was missing observations since November 2024; and one staff record was missing references.
The attendance was not accurate at the start of the visit. Two children had entered and were not included on the attendance until licensor asked to check the list.
The following was observed to be accessible to children: In the boys' bathroom, one of the metal wall plates behind the toilet was rusty and had sharp metal pieces.
One child's emergency medication to treat symptoms of an allergy was not on site (parent had provider the trainer EpiPen). Emergency or life-saving medications, such as asthma inhalers and epinephrine auto-injectors, for any children for whom they have been prescribed must be readily available at all times.
The medication consent for a child's emergency medication was not available. All medications must be accompanied by written parental consent and healthcare practitioner authorization when required, including proper administration of medication.

A review of four children's records indicated that one child did not have a completed Medication Consent form. The name of the child was shared with the Site Coordinator during the exit interview.

Toxic Substances
In the gym, a large floor fan was operating while the children were present. Electric fans, if used, must not be accessible to children.
The following was observed to be accessible to children: -In the gym, many of the metal heating pipe covers along the walls are loose and/or missing. When the licensor touched the end of one of the metal heating unit covers, approximately 20 feet of metal cover fell to the floor. It was dusty and rusty. -In the boys' and girls' bathrooms near the OT room, the parts of the piping to the heaters were uncovered and could become very hot during the winter when in use.
The following was observed to be accessible to children: -In the gym, a white plastic cover that covered up an old electrical outlet box was broken and had sharp edges. -In the boys' bathroom near the gym and kindergarten wing, rust was found on the wall vents behind the toilets. -In the boys' and girls' bathrooms near the OT room, rust was found on the bottom of the metal wall partitions and on the piping to the heating pipes.
The following was observed to be accessible to children: -On one of the windows on the back of the school, a piece of the thick metal screen has been cut out and the edges of the metal are sharp.
The following was accessible to children: -An open bag of ice melt was sitting inside on the floor at the exit used by the program.
In the cafeteria, two trash containers being utilized by the program were not covered as required.
