Staff:child ratios and groupings: Fixed; Toddlers
While walking through the toddler room to enter the infant classroom, it was noted that a toddler teacher was alone with five toddlers. The ratio is 4 children to 1 educator.
Documentation of medication administration; Current consent for medications
Six children's files were reviewed. One child's file contained an expired face sheet, consent for first aid, and transportation plan.
One medication consent form was missing the parent's signature.

An inadequate depth of mulch was observed around the climbing structures on the preschool playground.
Through an interview with staff, it was learned the program is still warming water in a microwave and placing it on the counter to warm infant bottles. This was cited on a previous visit.

A review of the preschool and pre-kindergarten refrigerators showed the following: * The preschool refrigerator did not have a working thermometer * The Pre-K refrigerator had a thermometer reading of 14.7 degrees Celsius (58.46 degrees Fahrenheit)
A kitchen play set was unsecured causing a tipping hazard.
Bottle Warming safety
While observing the infant classroom, it was noted water is heated in the microwave and then placed on the counter for warming infant bottles.
This renewal - monitoring visit inspection recorded no violations or advisories.
The following hazards where seen outdoors during the visit: 1) Children were allowed to use the climbing structures albeit the full 9 inches of the programs loose fill impact absorbing material did not meet the required depth as the ground was still partially frozen. 2) The landscaping tarp under the outdoor playground mulch was exposed causing a potential tripping hazard
Although an Educator was observed cleaning and sanitizing the snack table, this did not occur prior to the children sitting at the table and putting down their napkins. The Educator then asked all of the children to pick up their napkins and push their chairs back while she cleaned and sanitized the table, however the bleach and water solution was able to air dry for the required two minutes and was wiped clean to avoid having the children touch the solvent. The table where the toddler children ate their lunches was not cleaned and sanitized prior to the children sitting at it to eat.
The following indoor hazards were observed throughout the program: 1) An Educator's purse was observed as being accessible to children in the preschool classroom 2) A play “lemonade stand”, plastic dramatic kitchen set, and doll house were found unsecured in the pre-k classroom. 3) The trash container where children throw their food refuse in the pre-k classroom did not have a fully closed lid 4) The safety lock to one drawer in the preschool classroom was not working allowing small hazardous items and permanent/sharpie markers accessible to children 5) A dramatic play tool set was not secured in the preschool classroom 6) A dramatic play plastic puppet stand, and a book shelf that houses reading books was found unsecured in the toddler classroom.

A review for three staff files, showed two were missing evidence of references.
One student was not signed into the toddler attendance.
The following was observed in the- toddler room: 1. A chord hanging down form a shelf was within reach of the children. 2. the bookshelf was unsecured. The following was observed in the preschool classroom: 1. the bookshelf and play kitchen was unsecured. 2. The corner of the "road" rug was curled causing a tripping hazard.
A weighted sleep sac was observed i an infant's crib.
An educator was observed wiping a child's nose without washing their hands after.

A review of the staff records checklist showed three staff were missing evidence of physical and [REDACTED]one staff file was missing evidence of [REDACTED]
A review of the staff records and one staff file showed that one educator was working in the program without a completed BRC.
Four infants were observed with one educator. The ratio for infants is 1 educator to 3 infants. The program was cited at a previous visit for being out of ratio in the preschool classroom.
The building inspection is expired.
A plunger was observed stored within reach in the preschool bathroom. This was cited in a previous visit.
A review of emergency medications showed it was not readily available were the child to have an allergic reaction. This was cited in a previous visit.
A review of three IHCP's showed two were missing the required training information.
Broken and gaping lattice was observed around the base of the stairs and deck on the toddler playground.
A health and safety inspection showed the following: * A plunger was observed stored within reach of children in the preschool bathroom. * A "Glade Plug-In" was within reach of children.
Through an interview with staff, it was learned the emergency medication is not always readily available as they don't bring it with them when they go out to the playground.

Through a random review of three staff files the following was noted: 1) Two files did not have evidence of current professional qualifications 2) Two files did not have evidence of completed program orientation 3) One file did not have evidence of a current health [REDACTED] . 4) One file did not have evidence of [REDACTED]
An infant was not adequately supervised when a child was napping in a bottom crib on the gated off side of the room where all of the cribs are, while one Educator was on the other side of the room feeding a child in the rocking chair, and the second Educator was in the bathroom changing the diaper of a child. Although the Educator in the bathroom could see the side of the classroom where the Educator was seated in the rocking chair, neither Educator could see the napping child, based on their position and the position of this child's crib.
The [REDACTED] for an emergency medication for a child in the toddler classroom had expired on [REDACTED] .
Staff did not consistently wash their hands after changing children’s diapers or cleaning and disinfecting the diapering pad.
