Licensor randomly reviewed 6 of the children's records. Of these, 2 were complete and up to date, 1 had no medical info (physical or immunizations), 1 had an out of date physical and immunizations, and 2 were both missing a signature on pg. 6 of the enrollment packet, under Basic Transport.

Licensor inspected all of the licensed, indoor space during the renewal visit and observed the following: ** PLAYROOM **- no hazards ** LIVING ROOM **- stone hearth in front of fireplace needs to have edges padded/child proofed ** DINING ROOM **- no hazards ** KITCHEN **- no hazards ** 1st floor BATHROOM ** - hygiene products in bath that are lower than 4 ft and accessible to the children. ** 2ND FLOOR: PLAYROOM **- on the right side of the desk, 2nd drawer down, the child safety tab isn’t working and there are glue guns inside, there were also sharpies and scissors on the desk ** 1st FLOOR BATHROOM **- hygiene products in bathtub that are lower than 4ft and accessible to the children.

The temperature of the water in the bathroom was 130F degrees. Regulations state the maximum temperature must be 120F degrees.
Progress Reports
Space (indoor / outdoor hazards) - R Apr 2025
Licensor reviewed the children's progress reports. Of the 6 children enrolled, 3 were complete and up to date, but the other 3 were out of date.
Licensor inspected all of the licensed, indoor space during the monitoring visit, as well as the outdoor playspace. The only hazard that was observed was in the kitchen. The 2nd drawer down to the right of the stove had sharp knives inside and the child safety latch was not working.

All of the required items were observed in the first aid kit, except the medical tape.

Licensor reviewed 5 of the children's records. Of these, 4 had a complete and up to date progress report. However, the 5th record did not have a progress report since 2021. This was a school age child, so a progress report is required to be completed every 12 months.

OUTSIDE - fenced in area in back yard, no hazards
Supervision
Discussed written notifications to parents with the educator. Educator does not have the presence and location of firearms in the home in writing for the parents. Discussed adding this to the parent handbook.

First Aid Kit
Notifications to Parents - Written
The educator submitted a child record checklist. Reviewed 6 children's files during the visit. One child is missing a current Annual Update form signed by a parent. One child is missing a current physical and immunizations. Annual Update forms reviewed were signed in September 2021 or May 2022.
First aid kit has a CPR mouth guard, gauze, Band-aids, a touchless thermometer, etc. Kit is missing an instant cold pack. Most of the emergency card pages for the children are outdated and need to be re-signed and dated by the parents.
Upon arrival the licensor observed 6 children playing in the outdoor play space unsupervised. The educator had stepped into the house with two of the children to grab towels for children playing in a blow up wading pool and water table. Discussed the dangers of leaving children unattended outdoors, especially when they are engaged in water play. Discussed the educator making sure she has everything needed before going outside with the children or bringing all the children inside to grab towels. If the 7 year old's parents sign the indirect supervision form, the educator could even have had the 7 year old get the towels for her.

This investigation inspection recorded no violations or advisories.

Stairways
Indoor Space - free of hazards
In the living room the stairs to the second floor were not gated at the bottom. Attached gate at the top of the playroom stairs. Door at the bottom of those stairs.
