Educator is licensed to care for 10 children. There were 9 children present during the renewal visit, including Educator's 2 biological children. Due to the fact that Educator did not have an assistant working with her, Educator was over capacity during the renewal visit.
The emergency numbers/information are posted in the garage. Educator needs to add the contact info for the nearest emergency medical facility.
There were 9 children (including Educator's 2 biological children) present during the renewal visit and Educator did not have a licensed assistant present. The father of Educator's children was caring for them upstairs. However, Licensor explained that because they were in the home, they still counted in Educator's capacity.
Educator completed 30.5 hours of professional development, all in 2025. There were no training certificates from 2024 or 2023.

Educator uses the Tadpoles app right now for attendance, but is planning to switch to the Pro Care app. In reviewing the attendance, there were multiple days in which the parents did not sign their children out. Discussed that ensuring the children's time of arrival and departure are documented daily is Educator’s responsibility.
Educator has compelted a fire drill every month. However, the same exits has been used for every drill and Educator doesn’t record the length of time it takes to complete the drills.

The educator was caring for 7 children when the licensor arrived. Two of the children are her own 1 year old and 3 year old sons. The other children are aged 1 year, 3 years (2 children), and 4 years (2 children) old. One of the 7 children would have to be at least 5 years old (school age) in order for the educator to be in ratio. The educator's 16 [REDACTED]was present and brought the educator's own children upstairs into unlicensed space with him. The educator will need to submit a plan for how she plans to ensure that this situation does not occur again. The educator's [REDACTED] is not a licensed assistant and not even eligible to become one until he is 18 years old. Therefore, he cannot be caring for child care children with the educator. The educator's husband is her licensed assistant but was not on premises at the time of the visit.
Portfolio binders for each child that include their artwork, photographs, completed worksheets and progress reports. Educator uses Ages and Stages Questionnaires and a Kindergarten progress report. Reports reviewed were completed in October 2021, January 2022, May 2022 and June 2022. One child is overdue for a current progress report. Written progress reports must be completed every three months for infants and children with special needs, every six months for toddlers and preschoolers, and once a year for school aged children (5 years and older). Discussed a copy of the written reports being sent home with the parent and the educator keeping a copy in the child's file at the program.
Observed a variety of hazards in the outdoor play space that is currently under construction. * construction tools and a wheelbarrow in the yard. * plastic piping coming out of the ground in various places. * several large piles of dirt and heavy rocks. * mulch area with a tarp underneath has uneven parts and holes. * multiple long wooden boards on the ground. * many large concrete blocks. * drop off in the yard is not properly barricaded with a fence. The educator stated that she is currently not using the backyard with the children due to these hazards. Due to these hazards, the backyard will not be approved for use at this time. Also, the front yard is not approved for use due to a rock wall hazards. Educator must explain how she keeps the children away from these hazards and how she provides children with outdoor/physical activity daily.

Evans is required to complete 5 to 10 hours of training per calendar year depending on how often he works with the educator. Current Medical Form is missing. [REDACTED]has not yet completed the required 12 EEC Essentials trainings on the Strong Start website.
The refrigerator temperature was 20 degrees. Temperature must always be between 32 and 40 degrees.
Slide propped up against a rocky hill in the yard to create an incline of over 18 inches. No fall zones underneath the slide. Slide must be secured according to equipment regulations, fall zones must be installed or slide removed altogether. Drop off in the yard is barricaded with a fence. Deck is used for emergency exit only. Lattice installed around the bottom of the deck.

Outdoor Playspace - Hazards/Porches and Decks
One waterproof pad for diapering. Educator does not use a required disposable layer between each child and the pad. Educator wears gloves when diapering. Discussed the recommended use of a smock or oversized shirt as extra PPE.
Medication administration policy is included in the parent handbook. [REDACTED]. Educator stated that the [REDACTED] is brought back and forth from home. Discussed [REDACTED] being on premises at all times and stored in a labeled location where it can be easily accessed by the educator , but not the children. The [REDACTED] filled out by the child's [REDACTED] is not current and a new one must be completed and on file at the program. The parents have not signed the [REDACTED] Consent form which must also be kept on file at the program
