Licensor randomly reviewed 3 of the children's records. Of these, 1 was complete and up to date, while the other 2 both had out of date testing for lead.
Licensor inspected all of the licensed, indoor space during the visit, as well as the outdoor playspace (fenced in side yard). The only hazard that was observed was in the diningroom. There was an outlet exposed under the end table.

Licensor reviewed 4 of the children's records. Of these, 2 were complete and up to date. Both of the other 2 records did not have lead testing, and 1 of these records also did not have an enrollment packet.

There is a gate on the stairs that lead from the first to the second floor. However, the gate is an entrapment hazard as there is a 5 inch gap at the bottom, between the gate and the step.
Some of the equipment is too crowded together in the fallzone, and this has created a hazard as it would easy for children to get hurt.
