The child health screening form shall be available for review by the department for each child no more than 60 calendar days after the date of admission.

The licensing coordinator’s review of 10 children’s records revealed there was no physical examination on file for one child. Corrective action: The missing physical examination has been added to the child's file. Staff have been reminded to check that all required paperwork, including health forms, is complete before a child starts the program.

The licensing coordinator's review of five staff records for professional development completed in 2024 revealed that Staff A completed 3 hours and Staff B completed 8 hours rather than the 12 hours required by He-C 4002.33(f), in effect from April 21, 2022, through September 21, 2025. Corrective action: The program acknowledges that 2 staff members did not complete the required 12 hours of professional development for 2024. The 2 staff members have been reminded to complete the required development for 2025. Moving forward, the School Age office will monitor all staff professional development and send written reminders quarterly to ensure all staff remain on track.

The licensing coordinator’s review of the daily staff attendance records revealed on October 7 and 8, 2024, no staff signed in or out of the program and on the day of the visit two staff did not sign in when they arrived at the program. Corrective action: Moving forward, the program will have a checklist specific to staff signing in and out of the staff attendance record daily. Every staff will be required to initial the checklist acknowledging they signed in and out daily. The School Age Care office staff will initiate "spot checks" at the program to ensure the progam is in compliance with the corrective action plan.
