Programs serving diapered children and children who are not toilet trained shall have a diaper changing area, which shall be located adjacent to or in close proximity to a designated hand washing sink to allow access for hand washing without having to open doors or gates or have physical contact with other children.

The licensing coordinator observed that staff in the Winnie Way Classroom did not open the bathroom door prior to changing two children’s diapers which required them to open the door after changing the diapers so the staff and children could access the designated hand washing sink. Corrective action: Now and in the future the half door to the bathroom is opened prior to changing diapers.

With permission of the parent or guardian, the program shall post each child’s allergy care plan and treatment plan in the kitchen or food preparation area, the child's classroom, and wherever the child might have contact with the allergen(s).

The licensing coordinator observed that a child’s written allergy care and treatment plan was not available for review in the Black Bear Classroom. Corrective action: Now and in the future all action plans will be posted and staff will be aware of the plan. In the Black Bear classroom, the action plan is clearly displayed inside the classroom by the eating area.

Cribs and play pens required under (c) above shall: Not be stacked.

The licensing coordinator observed three playpens that did not have mattresses that fit properly and there was more than one inch of space between the mattress and the the playpen. Corrective action: All porta cribs have the correct mattresses placed inside each crib and do not leave a gap more than one inch. Now and in the future, we will ensure that all mattress meet licensing standards.

Any contaminated, expired, or discontinued medication, whether prescription or over-the-counter, and topical substances shall be returned to the child’s parents or, if belonging to the program, disposed of properly by authorized staff.

The licensing coordinator observed a child’s seizure medication labeled with instructions to discard the medication after May 26, 2023, in the Black Bears Classroom. Corrective action: Medication was given back to the parent. Now and in the future we will discard expired medications.

Non-toxic materials labeled “keep out of reach of children” shall only be used during a teacher-directed activity.

The licensing coordinator observed the following items labeled "keep out of reach of children" accessible to children; in the Baboosic Classroom there were two cans of shaving cream on a shelf 36 inches from the floor, and in the Granite State Extension Classroom there was Ajax Ultra next to the sink, as well as three cans of shaving cream in a bag on the floor. Corrective action: All items have been removed and will be in a locked cabinet going forward.

All center directors, agency administrators, site coordinators, or site directors, and all other child care staff who are responsible for the supervision of children, or who are necessary for the staff to child ratios, shall keep on file documentation of completion of a minimum of 6 hours of professional development, which shall be completed in accordance with the following: (1)  Within 90 days of the first date of employment; (2) Within 2 weeks for programs operating 3 months of the year or less; or (3) By providing documentation of previous completion.

Two licensing coordinators' review of 10 staff records revealed Staff A completed 13 ½ hours of professional development, Staff B completed 15 ¼ hours of professional development, Staff C completed 11 hours of professional development, and Staff D completed 7 hours of professional development in 2023, when 18 hours were required for each staff. Corrective action: The system that calculates professional development was unknown to us. All staff had 18 hours of professional development at the time of the visit. Because the previous calendar year was used as the method for assessing development hours, some staff fell short in the period that was being assessed. Staff are in full compliance now and will continue to do so in the future.
