Jacque Maiorana regular assistant. CPR expires on 3/2025 and first aid expires on 3/2024 Educator did not have a file for her assistant to reflect current physical and ongoing professional development trainings.
Staff Records
Educator has personalized parent handbook that does not include all required information. Educator will complete EEC’s parent handbook and upload it to document library.
Attendance records were reviewed. It was observed that on 11/10/23 none of the children were signed out. Educators is not keeping daily attendance records for her regular assistant.

Random review of children’s files was completed. It was observed that multiple files did not have annual updates. One file had an expired Individualized Health Care plan.
It was observed that the room in which children were resting was too dark not providing appropriate lighting for supervision.
There was access to standing water on the path to the outdoor play area. The standing water was on a large splash pad.
The outdoor area was observed to have hazards accessible to the children. It was observed that the temporary fencing installed to prevent access to the street on the right side of the yard was not secure as well as the fencing installed to prevent access to the swing set that is not approved for use of daycare children. It was observed that on the left side of the yard, where previously approved natural barrier was; seemed very sparse and no longer providing appropriate barrier to prevent access to the street. The educator has an approved fenced in play area, however also utilizes the patio for play activities, such as ride on toys, because of this; educator must install an appropriate barrier on the left side of the yard to prevent access to the street. There were two large pieces of glass on top of a patio table, which were loose and easily tippable. Patio chairs were observed to be turned over leaving the chair legs pointed straight up creating a safety hazard. It was observed that there were a number of random items being stored outside, which some were not accessible but others were, such as boxes with trash, and other items being piled up.

A random review of children’s files was completed. It was observed one child had no lead screening. Educator must submit children’s records checklist to verify she went through all her files.
It was observed that some of the progress reports did not have dates to indicate when they were documented. It was observed that some of the children are due for current progress reports. Discussed adding individual developmental goals for each child.
Educator had not documented fire drill from October and November 2023. Fire drill was observed during today’s visit.
There was a television on top of a toy shelf that is not secured.
Educator has two climbing structures. It was observed that there were some ride on toys close to one of the structures that prevented structures from having the 6 foot clearance from all sides.
Educator needs to update her list of medically involved children and ensure personal information is kept confidential.
