The following was observed to be accessible to children: The wooden kitchen set in the Elephants Classroom was no longer secure to the walls and is tippable. The Office door was unlocked and open and contains tall unsecure furniture, adults scissors on the desk and a paper cutter with sharp blade (unlocked).
The following was observed to be accessible to children: - The silver chain link fence on the outer area of the play yard measures at 34 inches high. This fence separates the play yard from a small body of water. Any such hazard must be removed or fenced by a sturdy, permanently installed barrier which is at least four feet high or otherwise protected or removed, as appropriate.

The licensee did not have documentation of the length of the staff meeting held in September 2020.
The licensee did not have evidence of observations of the educators while working with children at least every two months by a staff member with lead teacher qualifications or higher.
-One Individual Health Care Plan did not have evidence of who trained the educator to use the -medication. -One I.H.C.P. did not have evidence of which educators were trained to give the medication to the child.
The licensee did not have evidence of completing an emergency evacuation drill during the month of September 2020. The educators must hold practice evacuation drills with all groups of children monthly.
