Three staff records were reviewed during the visit and found one record was missing a bi-monthly observation between April 2025 and August 2025.
The following was observed during the visit to be accessible to children: in ROOM 6, a can of shaving cream was in an unlocked cabinet and in ROOM 4, a staple remover and multiple boxes of loose baggies were found in drawers.

The following was observed to be accessible to children: In ROOM 6, the wall is chipped under the window and the plaster and screws are exposed; and in ROOM 3, a hole is visible in the wall under the thermostat and the wall is chipped in three places to the left of the "Helping Hands" chart.

Three staff records were reviewed and found: One staff record was missing evidence 5 hours of professional development (I saw EEC Essentials 2.0 which is 12 hours and an additional 3 hour training). One staff record is missing professional development between hired date of 4/1/2024 and September 2024 (when the program begins their PD year), their PQ Registration expired on 3/20/3025, and a bi-monthly observation was missing for February 2025 (last observation conducted 12/2024). The third staff record had an orientation checklist that was not complete. It was not signed.
The following was found to be accessible to children: In ROOM 1, ROOM 6 and ROOM 4, adult pairs of scissors were found in unlocked drawers.

Three child records were reviewed and found: -One child’s record was missing medication consent forms for two medications.
The toddler and preschool spaces were observed and found the following is accessible to children: both outdoor areas have black cloth visible and emerging from the mulch which is a tripping hazard.
All IHCPs were reviewed and found they were all missing the names of educators who are trained to administer medication and who trained them.
