The following was observed to be accessible to children: In the Preschool area, black cloth was emerging from under the wooden structure and is a tripping hazard, the white rungs at the top of the wooden structure have a gap approximately 4 inches wide which is an entrapment hazard, and in both areas of the playground, the exterior gates were easy to open and would allow children to gain access to the parking lot if opened.

Six child records were reviewed and found all records were missing transportation plans. The licensee must maintain an individual written record for each child that includes: (a) Information Required at admission, including: consent for the child’s transportation plan, as required by 606 CMR 7.13(2)(e).
A review of three staff records was conducted and found staff member did not have evidence of completed EEC Essentials 2.0.
The following was observed to be accessible to children during the visit: In PREK O the kitchen set was not secure and tippable and three floor tiles were broken; in PRESCHOOL M two sets of cubbies were stacked and not secured to each other or to the wall and two books shelves were not secure and tippable; in PRESCHOOL N the kitchen set was not secure and tippable; in the Cooking Room, tape residue was on the floor, was sticky and not easy to clean; and in the internal/open spaced Gross Motor Room, the kitchen set was not secure and tippable.
The following was observed to be accessible to children during the visit: In TODDLER F, paint was chipping on the back right wall and in the bathroom next to PRESCHOOL M the wall paint was chipping and peeling behind the trash can.
The following was observed to be accessible to children: -In CHIPMUNKS, six holes were in the blue wall on the right side of the room and plaster was visible.
IHCPs were reviewed for all emergency medications and found: -Three children's IHCPs were not complete (missing who is trained to administer medication).
Five child records were reviewed and found: -One child's record did not have a current physical (last 10/20/2021), enrollment form, transportation plan, CPR/FA consent, or developmental history (expired 5/1/2024, 6/4/2024, and 6/8/2024). -One child's record did not have a current physical (last 4/26/2021), enrollment form, transportation plan, CPR/FA consent, or developmental history (expired 8/6/2023). -One child's record did not have a current physical (last 11/21/2022) and no progress reports since 7/12/2021. -One child's record did not have a current physical (last 11/21/2022 and no progress reports since 8/8/2022.
The following was observed to be accessible to children: -In CHIPMUNKS, a teacher's pocketbook was sitting on the edge of the counter. The pocketbook was moved during the visit to be out of reach of children. -In COYOTES, the toy kitchen furniture was not secure and tippable. -In WOLVES, the dollhouse was not secure and tippable.
The evacuation drill log could not be found during this visit.
One of the three staff records observed did not have evidence of a completed BRC.
Three staff records were reviewed and found: -One staff record was missing evidence of an interview, PQ registration expired 8/4/2021, and has a gap in bi-monthly observations from 9/2022 to 12/2023. -One staff record is missing evidence of a physical, is missing an observation from 4/2024 and did not have an EEC approved CPR/FA certification (took online only). -One staff record was missing evidence of two references, a current physical, evidence of MMR immunity, and completion of EEC Essentials 2.0.

One group of toddlers was observed to be without the children's water bottles while outside. The water bottles were observed in the classroom.
In COYOTES, two children's water bottles were not labeled with their names. Personal items intended for individual use by children, including but not limited to bottles, pacifiers, toothbrushes and sleeping materials, must be labeled with the name of the child for whom they are intended.
The following was observed to be accessible to children: -In WOLVES, the electrical outlet on the wall below the emergency bag was not flush with the wall. The top of the electrical box that fits into the wall was popping out towards the classroom approximately 2mm. All electrical outlets within the reach of children younger than school age must be made inaccessible by use of a safety device or covering that prevents access to the receptacle openings.
In the PRESCHOOL outdoor space, the following was found to be accessible to children: -The base for the birdhouse attached to the fence is splintering. -The sitting surface of the wooden bench near the climbing dome had split wood that could cause scratches or pinches to skin.
Evacuation procedures were not posted in the following locations: -On either side of the connecting door between Chipmunks and Coyotes.
One child's inhaler was not in the original box from the pharmacy.

Observations from original visit on 2023-08-02 00:00:00: Refrigerators were opened and thermometers observed and found: -In PIGLETS and WOLVES, the thermometer was missing. -In SEATURTLES, the thermometer read 41 degrees. -In STINGRAYS, the thermometer read 42 degrees. -In DOLPHINS, the thermometer read 80 degrees. -In SEAHORSES, , the thermometer read 50 degrees. Corrective Plan received on 2023-08-23 00:00:00: Refrigerators were given new thermometers and if still too warm, were taken out of the classroom. Follow up visit 1/30/2024: Five refrigerators were observed to be over the 32-40 degree reading as required. The thermometer in PIGLETS read 50 degrees. The thermometer in SHARKS read 42 degrees. The thermometer in COYOTES read 50 degrees. The thermometer in PONIES read 50 degrees. The thermometer in DUCKS read 42 degrees.

Corrective Plan received on 2023-08-23 00:00:00: Maintenace ticket was put in and is scheduled for all chipped paint. We can cover with a poster for now. Follow up visit 1/30/2024: In the MOTOR ROOM, holes were visible on the green walls beneath the phone and near the changing table. -In SEAHORSES, the edge of the middle wall is chipping and plaster is visible and the blue wall below the window is chipping paint. -In COYOTES, there is a crack in the wall under the window on the right.

One child's medical needs were not posted and another child listed was no longer assigned to the classroom. -The allergy list was missing from FOXES. Corrective Plan received on 2023-08-23 00:00:00: All allergy lists were updated and ensured that they were posted in the appropriate place in classroom. Follow up visit 1/30/2024: The list of medical alerts was missing from the WOLVES classroom.
Observations from original visit on 2023-08-02 00:00:00: Nine child records were reviewed and found: -One child's IHCP was not complete (missing name of staff trained) and did not have a current physical (last- 7/20/2022). -One child's enrollment forms were not current (expired 7/27/2023) and the IHCP was not signed/dated by parent. -One child's record was missing an IHCP for a chronic condition and did not have a current physical (last- 7/27/2022). -One child's record has an incomplete IHCP (missing staff trained). -One child's record has an incomplete IHCP (missing staff trained). -One child's record does not have current enrollment forms (expired 9/2022), the First Aid/CPR consent expired 9/2022, and the IHCP is not complete (missing who trained). Corrective Plan received on 2023-08-23 00:00:00: All IHCP were updated. Parents were notified of expired physicals. All families who still needed new enrollment forms were emailed and they will be in by this week due to new school year starting next week. Follow up visit 1/30/2024: Four IHCPs are incomplete and one IHCP was used for two medical conditions.

Corrective Plan received on 2023-08-23 00:00:00: Evacuation procedures were added to the new evacuation forms. Follow up visit 1/30/2024: Specific procedures to move infants and toddler out of the building safe and effectively were not added to the evacuation procedures.

Observations from original visit on 2023-08-02 00:00:00: Four staff records were reviewed and found: -One staff record had incomplete reference checks (the date of contact was missing and the name of one of the contacts was missing); no evidence of immunity to MMR; CPR and First Aid had expired 6/2023 and no bi-monthly observations. -One staff record was missing a reference check, did not have a resume or application, no interview was not dated, and was missing bi-monthly observations (last one dated 12/27/2022). -One staff record was missing evidence of an interview and references, PQ registration expired 4/26/2023, and no documented professional development in the last year. Corrective Plan received on 2023-08-23 00:00:00: All staff records were updated. Forms stating that hiring was done prior to acquision to Cadence were added to the files that had no documentation of hiring processes. Staff sent PQR ticket into the system to assist in logging in to system. Professional development from the prior year was added to files. This is documented through our employee system. Follow up visit 1/30/2024: -One staff record is still missing evidence that they renewed their PQ registration.
Observations from original visit on 2023-08-02 00:00:00: The following was observed to be accessible to children: -In PIGLETS, the unlocked drawer under the changing table contained loose plastic bags and gloves. -In SEA TURTLES, the two unlocked drawers under the changing table contained plastic bags of wipes. - In the unlocked HALL CLOSET across from the adult bathrooms, an extension cord, cleaning products, a broom, and a large uncovered trash barrel were found. -In STINGRAYS, the unlocked drawers under the changing table contained an open box of gloves loose plastic bags and a plastic bag of wipes and an educator's pocketbook was on the floor to the left of the refrigerator. -In SHARKS, an open box of gloves was observed on the edge of the changing table and a pocketbook was hanging on a low shelf in the closet. Both items were moved during the visit. -In FOXES, drawers contained two packages of alcohol wipes and a container of thumb tacks. These items were moved during the visit. -In WOLVES, a drawer near the sink contained an open box of gloves and sandwich bags. They items were moved during the visit. -In DOLPHINS, a hole the size of a quarter was in the green wall and plaster was visible and sticky residue from tape was on the top of the wooden bookshelf. -In the BATHROOM between DOLPHINS and SEAHORSES, unlocked drawers contained an open box of gloves, three skin ointments, and seven bug sprays. The ointments and bug sprays had labels that state, "Keep out of reach of Children". The unlocked cabinet in this bathroom contained plastic bags of wipes. -In SEAHORSES, the drawer to the left of the sink contained an open box of gloves and loose plastic sandwich bags, and a Magic Eraser cleaning sponge. These items were all moved during the visit. -In BEARS, a large container of antibacterial wipes was found under an unlocked cabinet. This container was moved during the visit. -In COYOTES, two holes were visible in the yellow wall at knee height and plaster was visible, and in the bathroom, a plunger sat next to the toilet, a dustpan and broom leaned against the wall, and a plastic bag of wipes sat on the back of the toilet. -Throughout the program, ceiling vents appeared to be dusty. -In all classrooms with refrigerators, the inside of the refrigerators appeared dirty as rust, stickiness, crumbs or black streaks were visible. Corrective Plan received on 2023-08-23 00:00:00: Piglets: locks were added to the drawers, and items were moved to higher shelf. Sea Turtles: locks were added to drawers, and items moved higher. Hall Closet: Lock added to door. Stingrays: locks were added to drawers, and items were moved. Teacher was reminded that pocketbook needs to be in locked cabinet or closet. Sharks: Teacher was reminded that personal items need to be up high. Lock was added to door, items were moved to a higher shelf or locked drawer. Foxes: Staff was reminded of items that need to be kept out of reach of children, tacks were removed from the classroom. Wolves: Items were permanently moved to higher shelf. Dophins: Maintenance ticket was made for hole in wall, sticky residue was removed. Bathroom SH/D: all items were removed and added to a higher cabinet or shelf out of reach of children. Staff was reminded of what can not be in reach of children. Drawers were secured with locks. Sea Horses: Items were permanently moved to a higher and locked shelf. Bears: Item was permanently moved to a lock cabinet Coyotes: Maintenance ticket was submitted and is scheduled. Items from bathroom was removed and broom holder was put up higher on wall for it to be off the floor. Teachers were reminded that wipes need to be up high in the shelf out of reach of children. Vents: Maintenace ticket has been submitted and we are awaiting date for cleaning. Refrigerators: cleaned and sanitized. If not able to clean properly they were removed from the classroom. Follow up visit 1/30/2024: -In FOXES, the unlocked cabinet under the sink contained bug spray and sunscreen which have the label "Keep Out of Reach of Children". -In the DOLPHINS BATHROOM, triple paste and wipes in plastic packaging were found in the unlocked cabinets under the sink. -In the open storage area across from Stingrays, the blue boat was standing up and leaning on other furniture and was not secure and tippable.
The attendance in PONIES was incorrect. Three children were not signed out at the time of the visit.
Observations from original visit on 2023-08-02 00:00:00: All emergency medications were viewed and found: -Two EpiPens were expired. One expired 3/2023 and the other expired 6/2023. Corrective Plan received on 2023-08-23 00:00:00: 3/3023 epi-pen was updated right away but student has since aged out of the program. 6/2023 epi-pen- Parents are in touch with the doctor and will have a note by the end of this week. Child is being tested again because doctor believes he is growing out of the allergy. Follow up visit 1/30/2024: Two emergency medications were not available in the classroom the children were assigned to on the day of the visit. Both medications were found by Licensor in the children's classrooms they normally attend but when they were reassigned to new classrooms for the day, their medications did not follow them. Licensor brought the medications to the classrooms where the children were for the day. A third child's emergency was not available in the classroom. -In two classrooms, children were sleeping on cots directly in front of closet that contained the classroom emergency bags with emergency medications.

Evacuation procedures were missing from the following spaces: -In all classrooms with shared bathrooms, evacuation procedures should be posted at those egresses as they are another way to evacuate the room. -At the exits leading from the hallways to outside, evacuation plans must be posted. -In WOLVES and COYOTES, the evacuation procedures were missing from the second egresses.
Evacuation procedures were reviewed in classrooms and found: -In CHICKS, specific evacuation procedures for infants and toddlers was missing from the evacuation procedures posted.
In FOXES, the educator working stated there wasn't any chronic medical conditions, however, the Director reported during the exit interview there are medical conditions in that classroom.
The following was observed: -In STINGRAYS, separate sinks are not being used for handwashing and rinsing dishes. -In WOLVES, a bucket containing dishes sat on the sink intended for handwashing. Facilities used for hand washing after diapering or toileting must be separate from facilities and areas used for food preparation and food service.
Refrigerators were opened and thermometers observed and found: -In PIGLETS and WOLVES, the thermometer was missing. -In SEATURTLES, the thermometer read 41 degrees. -In STINGRAYS, the thermometer read 42 degrees. -In DOLPHINS, the thermometer read 80 degrees. -In SEAHORSES, , the thermometer read 50 degrees.

The Preschool Bathroom did not have a Toileting and Diapering Plan posted on the wall as required.
The licensee did not have documentation of completing an emergency evacuation drill in the month of January 2023. The educator must hold practice evacuation drills with all groups of children and all educators from each floor level of the approved space at least monthly. Drills must be held during different times of the program day, and must use alternative exits. The educator must document the date, time, exit route used, number of children evacuated and effectiveness of each drill.
