A review of nine children’s records showed that all eight enrollment packets were expired. In addition, two physical examinations were expired, and one child had a visit summary on file in place of a required physical examination.

During the visit it was observed one child with allergies that was not posted.
During the visit, it was observed that there is a child with a documented allergy; however, there was no signed medication consent form on file to allow medication to be administered in the event of an allergic reaction.

It was observed that one file did not have an individualized health care plan for medical condition indicated on physical.
It was observed that for both children that have life saving medication prescribed, both did not have the prescription and one was not in its original container.
