Licensor observed pieces of broken glasses, a mirror, and other hazards within children's reach in the backyard. Educator’s outdoor space is not approved for use. Educator brings the children to Fallon Field.
Kitchen: the cabinets and drawers contains different types of spices, bag of screws, sharp and cutting objects, oils, cleaning products and other hazards in them and the safety lock is in disrepair.

Stairways
The back porch is missing a gate at the top of the stairs. The assistant put up a secure gate at the top of the stairs during the visit.
A tower of cubbies in the Livingroom is not secured to the wall and could be a tipping hazard. The assistant attached L-brackets to the cubbies during the visit and to the wall and the cubbies are now secure.
There is chipping and peeling paint on the front steps, front porch and railings and on the back porch and railings.

Outdoor space has a hockey table on its side with large wooden legs leaning against it which could be hazardous to children. There are a few green mats in the outdoor space that have large tears in them, making them a possible tripping hazard for children.
The educator informed the licensor that she is using one playpen to sleep two different children, children sleeps in the playpen at different times but per regulation each child must have their own sleeping material.
The educator does not have the parent permission to administer medicine on the child with a [REDACTED].

The First aid kit is missing a tape and two children's emergency cards are due for update.
The educator is using ASQ and Infant and Toddler progress report form for progress report. Licensor reviewed some of the children's progress report form and observed that educator is not completing children's progress report as regular as the regulation requires.

The educator is not documenting the children’s attendance accurately, parent are signing the form but are not logging the time children arrive or leave the program. Educator could not provide the licensor with the attendance list for this week.
