A review of five children’s records showed that one progress report has not been completed.
During the visit, it was observed that in the playroom, a toy storage shelf was not properly secured to the wall. In the kitchen, a drawer was observed to be unlocked containing items including, but not limited to, cords, screwdrivers, and batteries. In the bathroom, multiple shower and personal care products, along with a foot scraper, were observed on the shower floor. Additionally, a shelf located next to the toilet contained various products, including a prescribed cream, which were accessible to children.
Educator completed an online only CPR and First aid renewal certification. Educator will need to register and complete CPR and First Aid certification with valid hands-on skills portion.
It was observed that there was access to plastic bags, these are the bags in which diapers are kept. There was also access to diaper cream and baby wipes.
There were two children that needed an updated progress report.

Children’s files were reviewed. Five files need the annual update form. Two files were missing physical description on first page of enrollment packet. There is one child that does not have a complete enrollment packet on file.

Educator had a climbing structure indoors (school bus slide) that is not approved for indoor use must only be utilized outdoors. There was access to the outdoor swingset that is not approved for use.

Indoor Hazards
Children’s records checklist was reviewed prior to visit. Children’s files were reviewed during visit. Three files were observed not to have annual updates, one file did not have emergency card signed and dated by parent, general permission in five of the files was not completed with details on where children are taken when off premises. Two files did not have date of admission documented on face sheet.
Outlets in living room were not covered. Educator has requested to have this space added to her licensed space.
Educator had a climbing structure in her playroom. This structure measures above 18 inches in height and is currently placed on top of a carpet and a foam mat. Educator indicated this is an indoor structure but did not have specific information on this structure. This will be reviewed with FCC supervisor.
It was observed that the educator installed a swingset without the required ground surfacing and fall zone requirements. This structure is not approved for use. It was also observed that although the trampoline is not utilized with daycare children, in the way it is positioned it does give children access to the net opening.

The educator did not have her CPR certification on hand. The educator is required to submit proof of current CPR certification. First Aid expires on 8/2020
