Current consent for administered medications
A review of the refrigerator temperature showed a reading of 48 degrees Fahrenheit. Licensor informed the educator that refrigerator temperatures must be maintained between 32 and 40 degrees Fahrenheit.
Educator has two children in care with allergies but does not have medication consent form.

Refrigerator Temperature
A review of five children’s records showed two children did not have dates of admission listed, one general permission form was incomplete, and one physical examination was expired.
A review of the first aid kit showed that 10 emergency cards were expired. Licensor encouraged the educator to review all emergency cards and ensure each one is updated.
Required postings were observed with faded numbers that were not clearly legible.
The educator did not have the proper required posting. It was observed that the ICPH form was posted instead of the required form.

There was a toy shelf in the play room that was easily tippable.
It was observed in one of the family rooms, there was a storage shelf with gameboards that was easily tippable. There was a shelf within napping area in living room that was easily tippable. There was another storage shelf by the small pack and play which poses a safety hazard due to the stacked materials on top of the shelf, which included two glass jar candles and other small plastic bins. It was observed that table used for table activities and eating activities had a variety of materials that is cluttering and limiting the open space.

Napping Materials
Parent handbook could not be reviewed, educator did not have a hard copy easily accesible. Educator will submit copy of parent handbook.
It was observed there was a large cardboard box on top of a diapering table that posed a tipping hazard. It was discussed that the educator has an abundance of play materials for the children, however due to the abundance of materials some are not adequately organized in a way that makes it easy for the children to be able to observe materials accessible.
It was observed there was a pack and play stored inside another pack and play, which is not a sanitary manner to store napping materials.

Three children were observed napping on separate sofas. It was observed that not all children had an individual sheet to ensure children do not lay directly on the sofa and sofa pillows did not have individual pillow covers as well.
It was observed that there was a snow shovel accessible to children on the deck. It was observed that the cat food and water bowl were accessible to children in the kitchen.
