During the visit, the Allergy/Emergency Medication List posted in the Preschool Classroom was reviewed. One child attending was listed as having(REDACTED). The program did not have an Individual Health Care Plan as required for this child.
The diaper pail located in the Infant classroom did not have a cover. Soiled disposable diapers must be placed in a closed container that is lined with a leak-proof disposable lining.

During the visit, three children's files were reviewed. One file did not include a Developmental History Form. One file included a Developmental History Form dated 12/3/2023. This form must be updated annually.

In the Young Toddler Classroom, peeling paint was observed on the baseboard wood work. Three plastic packages of pull ups and three plastic pouches of diaper wipes were accessible to the children in the upstairs bathroom. Jugs of paint were stored on a shelf in the hallway and were not secured.
The garbage barrel in the Young Toddler Classroom did not include a cover.

A walk through of the outdoor space indicated the following: -On the front of the building where the "A,B,C, 1, 2, 3" sign is has peeling paint. -In the back playground, there is rotting wood and peeling paint near the gutter adjacent to the condenser unit. -The fenced area around the condenser unit has peeling paint.
Upon a review of three Children's Records at the program: -One child's Record did not have a current Medication Consent Form for both (REDACTED). -One child's Record did not have a current Medication Consent Form for a (REDACTED). This means in the event of a potential emergency, the program did not have consent to administer life saving medication.

During a walk through of the facility, the following was observed: -In the Toddler room there is peeling paint on the wall under the window next to where children's backpacks were hanging. -In the Preschool room there was chipped and peeling paint under the window next to the "Days of the Week" poster, under the green bulletin board, and along the trim boards of the room. -In the entry hallway there is chipped and peeling paint on the left side doorway trim board. -Under the orange front door of the building there is chipped and peeling paint.

Upon a walk through of the outdoor playground space, the following was observed: -There are several 3-4" inch gaps in the top portion of the chain link fence that need to be repaired. -There are two sections of fencing in which the vertical fence post is not secured to the top rail or post cap. -The bottom gate post hinge is not connected to the gate frame. -There are three broken pieces on the blue siding of the building near the "1,2,3" numbers that are approximately 4-12" wide and are in need of repair.
