Upon a review of three Children's Records reviewed during the visit, the following was identified: -One Child's Record did not have a parent signature and date on a Medication Consent Form authorizing the program to administer the child's (REDACTED) in the event of a potential medical emergency.

Individual Health Care Plan (IHCP) - R Apr 2025
Upon a review of three Children's Records during the visit, the following was identified: -One record did not include a complete Individual Health Care Plan (IHCP) for a child (REDACTED).
Upon a review of medications stored at the program during the visit, the following was identified: -One child's (REDACTED).
Upon a review of three Children’s Records, -A Medication Consent Form for a child’s prescribed (REDACTED) was incomplete. -A Medication Consent Form for a child’s prescribed (REDACTED) was not current and was signed on 2/5/2021. -A Medication Consent Form for a child prescribed an (REDACTED) was not signed by the parent/guardian. This means in the event of a potential medical emergency, three children did not have current and complete Medication Consent Forms allowing the program to administer medication.
Upon review of three children’s Individual Health Care Plans (IHCP), -One IHCP was incomplete and did not include the name of the educators in the program trained on the child’s medical condition or the name of who trained those educators. -One IHCP was incomplete and did not include required information about the symptoms of the medical condition, side effects of treatment, or potential consequences if treatment was not administered. -One IHCP was not current and was signed and dated on 2/10/2021.

Upon a walk through of the cafeteria, there was no posted evacuation route on the exit doors to the right of the adult desk.
Upon a review of medication stored at the program, -One child’s prescribed (REDACTED) expired in(REDACTED).
Upon review of four Children's Records, one lacked parental consent on a Medication Consent Form for a child that was (REDACTED).

Upon review of four Children's Records, three lacked complete Individual Health Care Forms (IHCP's). Each IHCP must include information on which educators in the program have been trained on the child's medical condition and who trained those educators.
A review of four children’s files indicated the following: -One file did not have evidence of a current physical examination (REDACTED). -One file had a parent signature on all required forms but did not have a corresponding date.
-The Building Inspection certificate expired on 9/1/18. -The Fire Inspection certificate is expired, with the last inspection certificate dated 6/28/18.
