The provider was not completing injury reports and providing them to parents for each minor injury.
There were no current medical forms on file for the staff members. Current CPR was not on file for one staff member.
Upon the licensor's arrival the provider was alone with seven children because provider allowed the assistant to leave the premises during rest time.
The tall drying rack in the small play room was not secure. The television on the shelf in the playroom was not secured.
