The wooden gate was broken to the outdoor play area.
The provider exceeded adult child ratios. There were five children present with the provider. Four of the children were under two years of age. This issue has been previously cited, 5/2013. Please submit children's schedules until the fourth child turns age two on 3/4/24.
The room was too dark to observe the children adequately in the port a cribs. In addition, one child was resting in a room with the door shut tight. This issue has been previously cited, 5/2013.

Children's hands were not washed with soap and running water prior to eating lunch.
There were cluttered items and materials in the exit route from the basement space.
Rabies documentation was not available for two cats.
There was a wood shelf in the bathroom that was not secured.
One child, (REDACTED), was drinking from a bottle at the table. The provider did not hold the child while drinking from the bottle.
There was a rusted see saw that needed to be removed from outdoor area.

Provider requested to use additional space in basement during visit. A LEAD transaction must be submitted.
The basement back exit had clutter and items in and along pathway that needed to be cleared.
This renewal - full review visit inspection recorded no violations or advisories.
Part of the basement was accessible to children behind the bathroom toilet and sink and was not blocked off with a secure barrier at least 4 feet in height. The furnace area and tool area were accessible in the basement exit and were not blocked off with a secure barrier at least 4 feet in height.
