Children's Records
Upon a review of three Children's Records, the following was identified: -One child's record did not include evidence of a Lead screening.
Upon a review of Staff Records at the program, the following was identified: -One Staff Record did not have evidence of MMR.
Upon a walk through of the cafeteria, there was not a posted emergency/evacuation procedure posted at each exit.
Toxic Substances
During a walk through of the cafeteria, three spray bottles with unknown liquid were accessible to children. One of the spray bottles was labeled “Mop Oil.” The other two spray bottles were not labeled. The Designee in charge stated these belonged to the public school custodian.

A review of five children’s files indicated the following: -One file did not have evidence of a current physical examination.
The Building Inspection certificate expired on 9/1/18.
During the visit, the following were observed and accessible to children: -In the cafeteria, there was a grey cabinet with materials for the children. On the lowest shelf of this cabinet were two spray bottles with a label of “Sanitizing.” These two spray bottles were relocated during the visit so as to no longer be accessible to children.
