All of the children's records were reviewed, two children with allergies did not have an individual health care plan.
In the indoor space, a charging wire was loose and accessible to the children. The first aid kit included two epi-pens which were accessible to the children under the sink in the bathroom. In the outdoor space, a large tarp was hanging over the fence with sitting water and two other toys also had sitting water.

The educator did not have the attendance documented for today. The educator completed the attendance for today during the visit. The educator is not accurately documenting the time in/time out time for two children who attend school part time during the day and return to the program. The educator did not document the attendance of one child present in the program on 10/06/25, 10/17/25 and 12/08/25.
In the bathroom, the cabinet under the sink had a safety lock but it was not functioning allowing access to personal hygiene items, air freshener, lotions and cough drops.
