At the time of the visit, in a review of three educators files, the documentation was not fully complete Staff A had an expired PQR. The program must maintain a personnel record for each staff member as outlined by the staff record checklist.

Current consent for administered medications
At the time of the visit, in a review of two students IHCPs, the documentation was not fully complete. Student C was diagnosed with two conditions and was missing documentation of the educators trained and the potential consequences if medication was not provided for one of their diagnosed conditions. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The plan must describe the chronic condition, its symptoms, any medical treatment that may be necessary while the child is in care, the potential side effects of that treatment, and the potential consequences to the child's health if the treatment is not administered.

At the time of the visit, in a review of two students IHCPs and the corresponding medication consent documentation, the documentation was not fully complete. Student B was missing the name and number of the prescribing health care practitioner.
Emergency medications readily available
At the time of the visit, in a review of two IHCPs and the corresponding prescribed medication, all prescribed medication was not readily available. Student A was prescribed two medications for their diagnosed health condition, they only had one prescribed medication on site. Emergency or life-saving medications, for any children for whom they have been prescribed, must be readily available.
At the time of the visit, it was observed that the handrails for the indoor ramp located outside of the cafeteria had chipping and peeling paint. It was observed that the steps and the poles on the outdoor playground equipment had chipping and peeling paint. The licensee must maintain the interior and exterior of the program in good repair, free of chipping, flaking, or peeling paint or broken plaster.
At the time of the visit, in a review of one IHCP and the corresponding medical documentation, the Medication Consent documentation was not fully complete. The document was missing the following information: if the child had taken the medication before, the dosage of medication, the dates for the medication to be provided, the times for the medication to be provided, reasons for medication, possible side effects, directions for storage and the number and phone number or the prescribing health care practitioner. The licensee must maintain as part of a child's record, an individual health care plan for each child with a chronic medical condition, which has been diagnosed by a licensed health care practitioner. The educator may administer routine, scheduled medication or treatment to the child with a chronic medical condition in accordance with written parental consent and licensed health care practitioner authorization.

On 12/11/2023, the program reported an injury requiring medical attention. At the time of the injury, there were 29 school-aged children in one group, confirmed by the Program Administrator. This indicated that the group was out of ratio.

A review of three three Individual Health Care Plans indicated that one did not have evidence of current paperwork completed. The program was in the process of collecting it from the child's parent.

A review of the staff records checklist and discussion with the Licensee, not all staff have evidence of a current physical examination as they are still in the process of obtaining documentation.
It was observed that multiple cleaning brooms were located in the corner of the cafeteria. This was discussed with the Licensee.
