A review of three children’s records indicated that one did not have current enrollment paperwork.
The following was observed: 1) There were stained ceiling tiles above the large window. 2) In the CHILDREN'S BATHROOOM, it was observed that the left side of the bathroom sink was detaching from the wall, causing the sink to lean forward. The Licensee stated that there was no progress with the property owner. (This was previously cited 3/5/26 and 7/1/25).

A review of the evacuation drill log indicated that a drill was no evidence of a drill completed January-March 2026. A drill was recently completed at the beginning of the month.

A review of three staff records indicated that one did not have evidence of a completed program orientation and a current physical examination.
It was observed in the CHILDREN'S BATHROOM that the side of the bathroom sink was detached from the wall, exposing pipes and there was a push mop and toilet plunger accessible to children.
The first aid kit and emergency bag did not contain cold packs. The Site Coordinator stated that more were ordered.

A review of the evacuation drill log from June 2024 to current indicated that there was no evidence of drills completed in July, Sept and Oct 2024.
During the visit, it was observed that there was an uncovered electrical outlet accessible to children.

During the visit, it was observed that there was cleaning products located above children's belongings with a warning label to "keep out of reach of children."
During the visit, there were preschool-aged children present with 2 Educators. The program is currently only licensed for 20 school-age children.

The building inspection recently expired on 6/21/24. The building department was contacted, and the program is waiting for an inspection.
It was observed that there were rust stains on the bathroom wall and stained ceiling tiles due to water damage.

During the visit, the following was observed and accessible to children: there was a ripped seam in the carpeting near the couch, the play kitchen and the white shelf were not secured and tippable.
A review of three children's records indicated that one did not have enrollment forms signed by the parent/guardian.

A review of five children's records indicated that three were missing a parent/guardian signature or date signed on enrollment forms.

The room thermostat read 64 degrees F. Room temperature in rooms occupied by children must be maintained at a minimum of 65 F.
During the visit, it was learned that the program was transporting children in their own vehicles. Whenever transportation is provided or contracted by the licensee the licensee must ensure that any program-owned, private, or hired vehicles such as sedans, vans, or station wagons used for the transportation of eight or fewer passengers, at any one time, and the driver thereof, conform to requirements as contained in M.G.L. c. 90, § 7(D), as applicable and any and all vehicles used for the transportation of children are registered and inspected in accordance with the laws of the state.
