The following was observed and accessible to children: 1) In the INFANT/TODDLER classroom, a corner of the base trim was peeling off the wall. 2) In the TODDLER/PRESCHOOL classroom, the play kitchen and three shelves were unsecured and tippable, and a corner of the base trim near the window was cracked and peeling off the wall. 3) In the CHILDREN'S BATHROOM, the toilet paper dispenser was not fully secured to the wall. 4) In the BACK HALLWAY outside the children's bathroom, the base trim was loose and a section of the dry wall was torn.

During the visit, it was observed that an Educator did not properly supervise three Toddler-aged children under their care when they left children unsupervised at least three times while EEC staff were present. Staff left 1 child in the hallway with licensors while they returned with the other children to their classroom. A second and third time the educator left the classroom leaving all children alone.
It was observed in the INFANT classroom that the handwashing sink was clogged with standing water. The sink for food prep was filled with children's toys.
It was observed in the INFANT classroom that an Educator did not follow diapering and toileting procedures by washing their hands or the child's hands after changing a child's diaper.

It was observed that there was no evidence of curriculum throughout the program (previously cited 9/22/23). During the visit, TVs were on in each classroom. The children in the INFANT classroom were watching Ms. Rachel and the children in PRESCHOOL 1 were watching a Ghostbusters cartoon.
The building inspection was expired as of 1/3/25. The Program Director stated that they are awaiting an inspection.
During the visit, the following was observed and accessible to children: underneath the hallway sinks, there was an exposed pipe and there were missing or loose baseboards outside the CHILDREN'S BATHROOM (previously cited 2/14/25, 2/21/24).

The following was observed and accessible to children: In the BACK HALLWAY outside of the children's bathroom, the vinyl baseboard was peeling and there were floor tiles missing. In the CHILDREN'S BATHROOM, the baseboard next to the right toilet was missing and the sink panel was peeling. There were stained ceiling tiles above the INFANT crib area. The window shade had cracked or missing panels. In PRESCHOOL 2, there was an uncovered electrical outlet and a chipped baseboard corner that was peeling.
It was observed that there were two children's cups not labeled with a name in PRESCHOOL 2.

A review of staff records indicated that two did not have evidence of a completed and current background record check.
The following was observed: -In the Toddler 2 classroom, there was one Educator alone with five children. The ratio for this age group is 1:4. -The attendance record in the Preschool 2 classroom was incorrect. This was immediately corrected by an Educator.
The following was observed and accessible to children: -In the Toddler 1 classroom, the manipulative shelf was not secured and tippable. -
In Toddler 2, one water bottle was not labeled with the child's name.
The following was observed and accessible to children: -In the Toddler 1 classroom, there was a bottle of cleaning solution on the play kitchen set, a broom and dustpan located next to the door, and plastic bags located on the side of the rocking chair. -In the Preschool 1 classroom, there was a vacuum and broom located next to the door, there were cleaning supplies located on the large desk. -In the Preschool 2 classroom, the emergency supply bag was on the floor next to the children's table. This was immediately corrected by an Educator. -In the children's bathroom, diapering supplies and plastic bags were located on top of or next to the toilets.
